ESSENTIAL FUNCTIONS:

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • Coordinate and facilitate patients’ mental health discharge plans.
  • Establish and maintain positive and productive relationships with community agencies that provide services to those with mental illnesses or other serious health conditions.
  • Assist medical and behavioral health teams in transferring patients to mental health facilities or emergency detention when necessary.
  • Adhere to security regulations of the client facility, maintaining confidentiality of patient information.
  • Initiate resumption of benefits process and assist patients in completing forms and obtaining personal identification documents necessary for transition.
  • Submit applications for patient financial assistance eligibility.
  • Document all services provided to patients in health records, including contacts with community providers after patient discharge.
  • Provide follow-up contacts after patient discharge and maintain tracking logs of each contact.
  • Partner with medical, behavioral health, and management teams to facilitate teamwork to collaboratively accomplish organizational goals.
  • Demonstrate commitment and dedication to providing patient support and guidance in their transition out of a correctional environment.
  • Maintain patient confidentiality and HIPAA practices as required by government agencies.
  • Assist in the training of new hires and coworkers.
  • Perform all other duties as assigned.

TRAINING EXPECTATIONS:

Associates in this role are expected, as a condition of employment, to complete all training assigned within a reasonable time period, generally within less than 15 days. This role includes the following training requirements:

  • Must maintain BLS/CPR certification.

EMOTIONAL INTELLIGENCE (emotional quotient; EQ):

To be successful in the organization, associates should have a high EQ. This is necessary to communicate productively, to build and maintain relationships, to recognize and reduce stress, to defuse conflict situations and to increase job satisfaction.

SUPERVISORY RESPONSIBILITY:

This is not a supervisory position. This position does not have the authority to hire, fire, discipline, discharge, assign overtime, and/or direct and assign work, with the ability to effectively recommend any of these actions. This role serves as a coach and mentor for other employees.

WORK ENVIRONMENT:

This job operates in a correctional facility or similar setting. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Incumbent will be exposed to viruses, disease, and infection from patients and specimens in the working environment.

PHYSICAL DEMANDS:

This position requires intermittent physical activity, including standing, walking, bending, kneeling, stooping, and crouching.

POSITION TYPE/EXPECTED HOURS OF WORK:

This position is scheduled based on operational need and may include rotational shifts, standby, as assigned, and on-call work. Evening and weekend work may be required as job duties demand.

TRAVEL:

This position occasionally requires travel.



Qualifications

REQUIRED EDUCATION AND EXPERIENCE:

  • Bachelor’s degree in a Behavioral/Social Science field (e.g., Psychology, Social Work, etc.) required
  • Combination of training, education, or lived experience working or participating in a program or organization serving system-impacted individuals required
  • Basic Life Support (BLS) or CPR training required
  • Knowledge and/or experience in correctional environments preferred
  • Knowledge of the criminal justice system preferred
