Job type: Per Diem

Hours: 0800-1630

The individual in this position works under the direction of the RN and/or Social Worker to complete referrals for post-acute services for patients and assist with tasks that do not require a clinical license or degree.

Physical demands:

• Frequent sitting.
• Frequent use of fine motor skill (typing/data entry). And reach with arms and hands.
• Frequent standing, walking.
• Occasional stoop, kneel or crawl.
• Lift and/or move up to 20 pounds and occasionally lift and/or move up to 50 pounds.

Working Conditions:

• May be exposed to infectious diseases.
• Interacts with patients and visitors.

Minimum Education/Certification/Experience:

• High School diploma or equivalent required.
• Bachelor’s degree preferred.
• Paramedic, EMT or Nursing Assistant license preferred.
• Minimum of 1 year Acute care experience.
• Skills required include excellent organizational skills, excellent verbal and written communication skills, demonstrated problem solving skills and computer literacy.
