About Connecting Families:

Connecting Families has provided essential support to children, young people, and adults nationwide for over a decade. We are committed to being industry leaders in providing best practice services and supports that embody our CARE values.

We are seeking compassionate and dedicated Disability Support Worker professionals to provide 24/7 assistance and support to our client with dementia in Ashmore QLD.

Responsibilities:

  • Assist clients with daily living activities such as bathing, dressing, and grooming
  • Provide emotional support and companionship to clients
  • Help clients with mobility and transportation needs
  • Administer medication and monitor health conditions
  • Assist with meal preparation and household tasks
  • Develop and implement individualized care plans
  • Collaborate with healthcare professionals and family members to ensure the best possible care for clients
  • Maintain accurate records and documentation

About you:

  • Demonstrated solid experience in supporting clients with dementia in either disability and/or aged care settings
  • Must be available to work across a rotating roster including weekdays, sleepovers and weekend shifts
  • Ability to manage difficult situations and behaviours and be able to de-escalate these situations in a therapeutic way

Requirements:

  • Minimum nationally accredited completion of Certificate III in relevant Disability or Community Service field
  • Current NDIS Worker Screening Check and Employee Working with Children Check
  • A valid Australian driver’s license with a reliable and comprehensively insured car
  • Current First Aid and CPR Certificate
  • Must be willing to obtain a new National Police Check
