About the role

As a Disability Support Worker, you will support NDIS participants to live as independently as possible while maintaining dignity, choice and control in their home environment. You will assist with daily living activities, promote community participation, and support individuals to work towards their personal goals in a structured, team-based support setting.

Participant overview

• Female, early 60s
• Psychosocial disability with significant cognitive impairment
• Secondary physical impairment impacting mobility, movement and balance
• Mobility can fluctuate significantly throughout the day
• High falls risk, uses a 4-wheel walker and, depending on presentation, may require a Sara Stedy/standing transfer aid or wheelchair
• Requires 1:1 assistance with most activities of daily living
• Requires support with showering, dressing, grooming and toileting
• Urinary incontinence and requires support with continence care and changing
• Requires assistance with meal preparation, household tasks and community access
• Requires prompting, task breakdown and support to initiate and complete daily routines
• May require additional time to process information and respond during conversations
• Can be apprehensive with unfamiliar people, so consistency and rapport-building are very important
• Can occasionally display verbal aggression when distressed and requires calm redirection and de-escalation
• Requires support with diabetes management under an appropriate nursing/delegated model of care
• Blood glucose monitoring and insulin administration form part of her daily health support needs
• Requires 24-hour support due to fluctuating mobility, falls risk, continence needs and assistance required across daily living activities
• Requires assistance with community access, appointments and meaningful activities

Key responsibilities

• Provide personalised support with daily activities such as meal preparation, medication assistance, personal care, and household tasks
• Encourage independence, social inclusion, and skill development
• Support participants with complex needs, including behaviours of concern
• Follow individual support plans and assist participants to achieve their goals
• Maintain accurate documentation and report any changes in participant needs
• Build respectful relationships with participants, families, and allied professionals
• Deliver support in line with NDIS Practice Standards and organisational policies

About you

• Strong experience in psychosocial disability and mental health
• Experience supporting people with complex mobility needs, Parkinsonism or neurological presentations
• Strong personal & continence care experience
• Manual handling and transfer experience; experience using a Sara Stedy or similar transfer equipment highly regarded; if not, training will be provided.
• Diabetes support experience highly regarded
• Confidence working within nursing delegation and following health management plans
• Strong observation and escalation skills
• Confidence recognising and reporting changes in presentation
• Calm and patient communication style
• Ability to build trust and rapport gradually
• Good understanding of behaviour support and de-escalation
• Strong documentation and communication skills
• Comfortable supporting community access and appointments
• Reliable availability and the ability to provide consistency of staffing

Compliance requirements

• Certificate III or IV in Disability, Individual Support or similar (or working towards)
• Current NDIS Worker Screening Check
• Working With Children Check
• Valid First Aid & CPR (HLTAID011)
• Current Australian Driver's Licence
• Valid Australian Working Rights
• Current National Police Check
• Victorian driver licence, registered vehicle and full comprehensive car insurance.
• Available for 3 full days (day and overnight shifts) per week, does not need to be consistent days.
