About Connecting Families:
Connecting Families has provided essential support to children, young people, and adults nationwide for over a decade. We are committed to being industry leaders in delivering best-practice services and supports that embody our CARE values.

We are seeking compassionate and dedicated Disability Support Workers to provide 24/7 assistance and support to our client with dementia in Ashmore, QLD

Responsibilities:

  • Assist clients with daily living activities such as bathing, dressing, and grooming

  • Provide emotional support and companionship

  • Assist with mobility and transportation needs

  • Administer medication and monitor health conditions

  • Assist with meal preparation and household tasks

  • Develop and implement individualized care plans

  • Collaborate with healthcare professionals and family members

  • Maintain accurate records and documentation

About You:

  • Demonstrated experience supporting clients with dementia in disability and/or aged care settings

  • Experienced in Manual Hoisting

  • Availability across a rotating roster including weekdays, sleepovers, and weekends

  • Ability to manage complex behaviours and de-escalate situations using therapeutic approaches

Requirements:

  • Minimum Certificate III in Disability, Community Services, or a related field

  • Current NDIS Worker Screening Check and Employee Working with Children Check

  • Valid Australian driver’s license with a reliable and comprehensively insured vehicle

  • Current First Aid and CPR Certificate

  • Willingness to obtain a new National Police Check
