Job Title: Director of Social Services

• College degree required.
• Licensed Social Worker may be required based on regulations and facility size.
• Skilled Nursing experience preferred.

Responsibilities:

• Plans, develops, organizes, implements, evaluates, and directs the social service programs of the facility.
• Participates in discharge planning, development, and implementation of social care plans and resident assessments.
• Interviews residents and families as necessary to identify medically related social and emotional needs.
