Kline Galland Home is seeking an experienced and compassionate Director of Social Services to join our leadership team on a full-time basis. This key leadership role is responsible for overseeing social services programs, resident advocacy, discharge planning, and care coordination for our long-term care facility.
We are looking for a dedicated professional with proven experience in long-term care who is committed to enhancing residents' quality of life, supporting families, and ensuring exceptional social services throughout the continuum of care.
Key Responsibilities
- Direct and oversee all social services functions within the facility.
- Advocate for residents' rights, dignity, and overall well-being.
- Lead discharge planning and care transition processes.
- Coordinate with interdisciplinary teams to ensure comprehensive resident care.
- Conduct psychosocial assessments and develop individualized service plans.
- Provide guidance, support, and counseling to residents and families.
- Ensure compliance with federal, state, and facility regulations.
- Maintain accurate documentation and resident records.
- Participate in care plan meetings, family conferences, and quality improvement initiatives.
- Serve as a liaison between residents, families, staff, and community resources.

Qualifications
- Required: Previous experience as a Director of Social Services in a Long-Term Care setting.
- Strong understanding of long-term care regulations, resident rights, and discharge planning.
- Excellent leadership, communication, and interpersonal skills.
- Ability to work collaboratively with residents, families, physicians, and interdisciplinary team members.
- Passion for resident-centered care and advocacy.
