Director of Social Services

$50,000-60,000 per year depending on experience

of Position

The Director of Social Services plays an integral role in the success of our team, and communities; creating a place our residents are proud to call home. The primary purpose of this position is to provide strategic leadership, establish departmental standards, and exercises independent judgment in managing operations, staffing, and budgetary performance of the Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Director of Social Services and/or Executive Director, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.

Essential Job Functions

  • Interview and complete written assessment of each patient within five days after admission to the services.
  • Development of a plan, including goals and treatment, for social work services for each patient who needs such services, with participation of the patient, the family, the patient's licensed healthcare practitioner acting within the scope of his or her professional licensure, the director of nursing services and other appropriate staff.
  • Weekly progress reports in the patient's health record written and signed by the social worker, social work assistant or social work aide.
  • Participation in regular staff conferences with the attending licensed healthcare practitioner acting within the scope of his or her professional licensure, the director of nursing service and other appropriate personnel.
  • Discharge planning for each patient and implementation of the plan.
  • Orientation and in-service education of other staff members on all shifts shall be conducted at least monthly by the social worker in charge of the social work service.
  • Perform other job-related duties as assigned

Personnel Functions

  • Assist in the recruitment, interviewing, and selection of personnel.
  • Schedule department work hours (including vacation and holiday schedules), personnel, work assignments.
  • Assist in staff development. Formally and informally monitor performance and ensure adjustments/corrections are made by using coaching, counseling, and discipline methods. Work with the Executive Director to resolve staff performance issues.
  • Develop and participate in the planning, conducting, scheduling, etc., of in-service training classes, on the job training and orientation programs for maintenance personnel.

Knowledge and Critical Skills

  • Be able to make independent decisions and follow instructions.
  • Deal tactfully with personnel, prospects, residents, family members, visitors, government agencies/personnel, and the public.
  • Capable of working with ill, disabled, elderly, and emotionally upset people within the facility.
  • Communicate effectively in a manner that is sufficient for effective communication with supervisors, team members, prospects, residents, and families.

Education and Experience

  • Must meet all applicable state and federal requirements for this position.
  • Bachelors of Social Work degree from a school of social work accredited by the Council on Social Work Education.
  • One or more years of social work experience in a health care setting.
  • One or more years of experience in a supervisor capacity

Supervisory Responsibility

This position has direct reports and supervisory requirements.

Working Conditions and Physical Demands

Tasks assigned to this position may involve potential and/or direct exposure to blood, body fluids, infectious diseases, air contaminants, and hazardous chemicals.

Physical Requirements - The Physical activities of this position involve:

  • Prolonged periods of sitting at a desk and working on a computer & telephone
  • Walking the property while touring, meeting with prospects and lease signings
  • Must be able to lift 15 pounds at times
  • Alternating between standing and sitting

Cognitive Requirements - The Cognitive activities with or without prosthetics of this position are:

  • Executes tasks independently
  • Ability to express yourself clearly and effectively
  • General commuter literacy

Environmental Requirements - An individual in this position may be exposed to:

  • Ambient room temperatures, lighting and traditional office equipment as found in a typical office environment
