What You'll Do:

• Provide clinical social work services to residents
• Support residents and families with emotional and psychosocial needs
• Collaborate with interdisciplinary care teams
• Assist with care planning and discharge planning processes
• Maintain accurate documentation and ensure compliance with regulations
• Promote and maintain high-quality, person-centered care

What You'll Need:

• Licensed Social Worker in the State of Nebraska required
• Master of Social Work (MSW) required
• 3 years of experience providing clinical services in long-term care preferred
• Equivalent experience in medical or mental health settings will be considered
• Must be 21 years of age or older
• Ability to understand, read, write, and speak English
