Care Alliance Health Center is seeking a Director of Quality & Population Health to join our team. This role is an opportunity to improve quality, patient safety and risk. About the Role

The Director of Quality & Population Health will reduce risk, advance health equity, and strengthen performance across attributed and underserved populations. This role will mitigate risk and drive continuous improvement.

What You’ll Do

In this role, you will:

• Lead the organization's quality improvement, patient safety, and quality-related, risk management strategy.
• Develop population health improvement strategies that drive care gap closure across clinical settings.
• Monitor quality and population health requirements across applicable payer and ACO arrangements.
• Oversee FTCA risk management requirements, including quarterly risk assessments, annual training, board reporting deeming activities, and organizational risk mitigation.
• Analyze quality, safety, population health and payer performance data to identify trends, variation, risks, and improvement opportunities.
• Use population health and performance data to identify patients requiring preventative screening, follow-up, outreach, or additional care coordination.
• Develop, implement, and maintain policies, procedures related to quality, patient safety, population health, and assigned risk functions.
• Provides direct leadership and supervision to the Quality Assurance & Risk Management Specialist and Cancer Screening Patient Navigators.

Work environment

This position operates in both clinical and administrative healthcare settings and requires regular interaction with leadership, providers, staff, patients, and external partners. Work is performed primarily in an office and outpatient healthcare environment with frequent use of computers, electronic health records, reporting systems, and other office technology. The role may require movement between departments and Care Alliance locations to support quality improvement, patient safety, population health, and regulatory readiness activities.

Requirements

What We’re Looking For

• Bachelor's degree in public health, Healthcare Administration, Nursing, Health Sciences, or a related healthcare field.
• Minimum five years of progressive experience in healthcare quality, and population health, value-based care, risk management, patient safety, regulatory compliance, or related healthcare performance functions, including demonstrated leadership responsibility. Demonstrated experience with population health and value-based care programs.
• Knowledge of payer quality measures, patient attribution, care gap management, preventative screening measures, and avalue-based performance requirements.
• Experience monitoring payer or ACO quality performance and developing interventions to improve results
• Experience with EHR data validation, clinical documentation, and quality reporting. Knowledge of quality improvement methodologies (PDSA, RCA, FMEA)
• Strong data analysis and reporting skills (Excel, dashboards, Power BI/Tableau preferred)
• Knowledge of regulatory and accreditation standards

Preferred

• Master’s degree in public health, Healthcare Administration, Nursing, Health Sciences, or a related field.
• Certified Professional in Healthcare Quality (CPHQ), Certified Professional in Patient Safety (CPPS), or Risk Management certification, or similar credential, strongly preferred
