Job Description

The Evergreen Center is a world-class private human services organization that provides living and learning environments for individuals with developmental disabilities diagnosed with multiple disabilities including Autism, physical disabilities, neurological problems and behavior disorders. The Director of Family Services and Admissions reports directly to

the Chief Executive Officer.

• Supervises Family Services Assistant, Family Support
  
  Coordinator, and Family Resources Coordinator.
• Maintains ongoing communication with parents/guardians.
• Provides resources and referral information for families and
  
  individual students for counseling services.
• Maintains Individual Family Service Records.
• Serves as a liaison during referral process.
• Maintains liaison with funding agencies and parents during
  
  intake process.
• Maintains liaison with funding agencies.
• Monitors progress with Chapter 688 referrals (maintains and
  
  monitors documentation).

Qualifications

• Master's degree in Social Work, Psychology, Special Education or related field
• Licensed Independent Clinical Social Worker (LICSW); Licensed
  
  Certified Social Worker (LCSW); Licensed Social Worker (LSW);
  
  Licensed Social Work Associate (LSWA) or mental health
  
  counseling license preferred
• Behavior Analysis (LABA) or (BCBA) preferred
• (5) Five years’ experience in the mental health or education field.
• (2) Two years’ supervisory experience and experience with developmentally or physically disabled persons preferred.
• Experience in a residential setting (preferred)
