JOB DUTIES/RESPONSIBILITIES

  • Forms effective working relationships throughout the organization. Uses these relationships to achieve the department's optimal performance. Conducts thorough investigation of pertinent complaints. Acts as a liaison between the staff and other hospital employees, patients, families, visitors and physicians as appropriate, ensuring the assistance provided is appropriate, helpful and reflects the positive attitudes and philosophy of the Hospital.
  • Models Standards of Behavior and ensures Standards are met by department staff. Establishes efficient and effective policies, practices, procedures and work rules that are consistent with the policies and philosophies of Gateway Regional Medical Center. Utilizes and promotes familiarization with hospital's mission statement and Standards of Behavior. Ensures HIPAA and confidentiality standards are met by the department at all times.
  • Actively participates on various committees relative to Social Service functions and responsibilities, and/or cross department/service performance assessment and improvement activities as requested, or as appropriate to meet the needs, mission and organizational plans of the hospital.
  • Monitors department processes, and implements necessary changes in a timely manner when processes are not working. Establishes accurate and effective systems for monitoring all departmental resources and services ensuring that established controls are reflective of the hospital's overall organization and approach. Informs Director of Quality of department activities, needs, problems and overall direction of its resources and services. Provide quality improvement statistics as requested with appropriate interpretation of data and action plan.
  • Provide support to all discharge planning staff. Provide Social Work, Care Coordinator, and Case Management staff direction regarding discharge planning and RCF/SNF placements. Collaborate with physicians regarding patients' progress and discharge planning. Educate patients, families and caregivers regarding available community resources. Assist and observe completion of placements, DA124ABC forms, 24-hour notice to facilities, weekly updates to facilities, guardian consent/paperwork, guardian contact, assist with the guardianship process and involuntary commitments.
  • Complete routine paperwork within prescribed time frames and efficiently document on individual progress notes all pertinent and meaningful observations and information. Provide oversight of appropriate documentation of groups and treatment planning to all psychiatric department staff. Provided social work support and interventions appropriate to discharge planning meetings on medical and psychiatric units.
  • Provides Director of Quality with daily social work census report including inpatient, outpatient, Emergency Department and Guardianship cases actively requiring social work consultation.
  • Provide assistance to patients with personal and environmental difficulties, which pre-dispose toward illness or interfere in obtaining benefits for medical or psychiatric care. Connect at-risk patients with Medicaid eligibility service and provide direction to patients/families needing assistance with disability benefits. Assist with other insurance eligibility requests as appropriate.
  • Provide support and guidance to outside Social Workers, Case Managers, Department of Health and Senior Services, Legal Counsel, and other case management entities as needed.
  • Report abuse/neglect issues to appropriate authorities. Maintain log of completed abuse/neglect checklists, ensure documentation is complete and responsible manager has signed.
  • Knowledgeable of Conditions of Participation/CMS, Joint Commission and Quality Management requirements and that staff documentation and practice meet these standards.
  • Encourage an atmosphere that fosters Social Work and Activity Therapists growth and development as well as retention.

LICENSES/CERTIFICATIONS

  • Master's Degree in Social Work required
  • A Licensed Clinical Social Worker is preferred
  • Active pursuit of Licensed Clinical Social Worker is required

EXPERIENCE

5 years management experience within the healthcare field preferred. History of employment or experience in an acute care field in a hospital environment.

  • Evaluative skills required for unit and patient assessments.
  • Decision making skills required for difficult problem solving.
  • Clinical reasoning and judgment necessary for evaluating the safety of the psychiatric units.
  • Excellent oral and written communication skills to interact with patients, visitors and staff.
  • Ability to recognize need for and alter communication with patients with communication impairments.
  • Ability to manage time, organize daily schedule to meet productivity standards, and cope with professional stress.
  • Must demonstrate the ability to interpret and apply medical and psychiatric social work concepts and principles.
  • Must demonstrate flexibility, intellectual imagination and maturity in the application of theoretical principles of practical problems of the patient.
  • Must have the ability to establish and maintain effective relationships with the patients, families or significant, medical, nursing, other social work staff or outside agencies involved in the care of the patient
  • Must have good verbal and written communication skills.
  • Must possess the skills necessary to assess risk issues in their assigned patient population.
  • Must have the ability to apply counseling concepts to their daily social work functions, i.e., group therapy, individual therapy, and assessment skills.
  • Has initiative consistent with keeping self-motivated to perform at peak levels at all times.
  • Has ability to recognize and give attention to many details at one time.
  • Has ability to prioritize job responsibilities in a fast pace and evolving/changing work environment.
  • Has ability to communicate effectively, both verbally and in written form, and preserves the confidentiality of the patients.
