Job Description


This role will provide supervisory oversight to Community Based Hospice (CBH) team including contracted services, pharmacy, DME and therapies. This position is responsible for ensuring that CBH team is in full compliance with all regulatory requirements and accepted standards of practice, and to act as a resource for day-to-day operations and patient care needs. This position will collaborate with the Vice President of Clinical Services on project planning, education of the IDG Teams related to policy and procedure, clinical process changes, new regulatory requirements, and accepted standards of practice. The Director of CBH will collaborate with CBH leaders by delegation to follow up on quality issues and audit results. This position will be responsible for identifying patients eligible for correct level of hospice care, (crisis care, general Inpatient care, routine care or respite care) in collaboration with CBH Team Leaders. The schedule for this position is Monday-Friday 8:00am-5:00pm; weekends and holidays as scheduled; manager on-call rotation

Director of CBH Responsibilities what you do:
Leadership, Culture & Accountability
• Engage in and support organizational efforts that promote diversity, equity, and inclusion across hiring, promotion, and employee relations.
• Set clear expectations, monitor performance, and ensure CBH leaders and teams are accountable for achieving organizational and departmental goals.
• Promote a culture of collaboration, engagement, and shared decision-making.
• Participate in strategic planning and design processes to achieve CBH and organizational goals
Clinical Oversight & Patient Care Excellence
• Ensure all patient care aligns with regulatory requirements, standards of practice, and patient/family goals of care.
• Serve as a clinical resource to CBH Leaders for complex patient and family situations.
• Ensure appropriate levels of hospice care are implemented (RHC, GIP, Crisis Care, Respite).
• Support IDG case conferences and interdisciplinary collaboration.
• Oversee triage and continuous care operations to ensure timely, appropriate responses to patient needs.
Quality, Compliance & Regulatory Oversight
• Maintain thorough knowledge of hospice regulations, ACHC standards, and applicable state/federal guidelines.
• Oversee compliance across all CBH services, including IDG teams, CNAs, Triage, contract staff, and Crisis Care.
• Serve as CBH representative during surveys and ensure readiness and follow-up on plans of correction.
• Monitor and analyze quality data (QI audits, CAHPS, Peer Reviews, feedback reports) and ensure corrective actions are implemented.
• Maintain required records, reports, and regulatory documentation in compliance with organizational standards.
Operations, Efficiency & Standardization
• Drive standardization of clinical and operational processes across CBH to ensure consistency, safety, and efficiency.
• Ensure staffing models and scheduling effectively meet patient needs and optimize resource utilization.
• Evaluate and optimize productivity and efficiency across all roles and service lines.
• Develop and implement process improvements to enhance care delivery and operational performance.
Team Development & Workforce Management
• Partner with the Vice President of Clinical Services in hiring, onboarding, performance management, and disciplinary actions.
• Ensure effective orientation and onboarding tailored to staff experience and skill level.
• Oversee annual education, competency compliance, and ongoing professional development.
• Support CBH Leaders in developing their teams and addressing performance or quality concerns.
Vendor & Contracted Services Oversight
• Provide oversight and accountability for contracted services including pharmacy, DME, and therapies.
• Collaborate with vendors to ensure service quality, performance expectations, and contractual compliance.
• Ensure contracted staff are effectively integrated into interdisciplinary teams and aligned with CBH standards.
Collaboration & Relationship Management
• Collaborate with internal departments (Quality, Education, IT, PI, etc.) to support clinical and operational goals.
• Maintain strong relationships and communication with community partners, including SNFs and ALFs.
• Collaborate across service lines with other leaders to ensure coordinated, patient- and family-centered care.
• Support customer service excellence through timely complaint resolution and follow-up.
Financial & Administrative Oversight
• Manage the CBH program budget and collaborate on team budgets with the Vice President of Clinical Services.
• Monitor operational performance and align resources to meet financial and strategic goals.
Additional Responsibilities
• Participate in community education and outreach activities.
• Attend professional development opportunities (seminars/workshops).
• Cover Administrator On Call (AOC) and holiday assignments as required.
• Perform other duties as assigned by the Vice President of Clinical Services.

Requirements (what you need):
• Registered Nurse with a current NC License.
• BSN preferred.
• Minimum of 5 years recent clinical experience, with at least 3 of which were in a successful supervisory role.
• Minimum of 1 years recent Hospice clinical experience
• Hospice Nurse Certification Preferred
• Able to independently seek resources and work collaboratively.
• Able to communicate clearly with patients, families, visitors, IDG Team, physicians, facility staff, and leadership / Executives inside and outside of Transitions.
• Able to educate patients and families regarding the hospice plan of care.
• Knowledgeable about hospice regulations, reimbursement, including Medicare / Medicaid requirements, licensing laws, and accreditation standards.
• Excellent written and verbal communication skills, and competence with computer systems / EMR
• Reliable transportation
• May be required to lift and carry up to 50lbs.
• Must have strong team Building skills.
• Ability to communicate effectively with team members, set clear expectations and lead by example.
