Mid: $137,155 (~10 years of experience)
Max: $150,000 (15+ years of experience)

A large Level I Trauma acute care hospital in Central Florida is seeking an experienced Director of Case Management to lead and oversee hospital-wide care coordination and discharge planning operations.
This is a senior leadership role responsible for clinical, operational, and regulatory oversight of case management services in a 204-bed hospital with high patient volume. The Director will lead a multidisciplinary team and partner closely with executive leadership to ensure quality outcomes, compliance, and efficient utilization of resources.
Provide strategic and operational leadership for all case management functions
Direct daily department operations, including staffing models, productivity, onboarding, and competency management
Ensure effective care coordination and discharge planning processes across all service lines
Monitor department performance through data analysis, quality metrics, and regulatory benchmarks
Lead performance improvement initiatives to enhance patient outcomes and throughput
Ensure compliance with federal, state, and accreditation standards
Collaborate with executive and physician leadership, including finance, nursing, medical staff, ethics, and legal partners
Partner with division leadership to achieve financial, quality, and operational goals
Foster a culture of accountability, collaboration, and professional development
Required:
Bachelor’s degree
Active RN, LCSW, or LMSW license
Minimum 3 years of acute care hospital case management experience
Minimum 2 years of case management leadership experience in an acute care setting
Recent acute care experience (within the past year)
Preferred:
Master’s degree in Nursing, Healthcare Administration, or Business Administration
Experience leading case management teams in large or high-volume hospitals
Strong regulatory, utilization review, and discharge planning expertise
