The Director of Care Coordination will:

  • Plan, implement, and administer comprehensive service delivery in case management, progression of care, social work services, discharge planning, and transitions in care.
  • Promote/establish effective communication between Care Coordination and other Hospital departments and members of the medical staff
  • Keep informed regarding current regulatory and professional trends, changes, and standards applicable to the scope of Care Coordination and make appropriate recommendations or changes in policies, procedures, and/or programs to conform to regulation changes and standards.
  • Keep staff informed of Hospital Organizational goals, strategic plans and initiatives, finances, and Senior Management communication so as to ensure identification with the overall Hospital mission and goals.
  • With Managers, establish effective lines of communication for the completion of objectives, oversight of areas of responsibility and to ensure consistency for all Department staff.
  • Oversee the continuous assessment and improvement of the care coordination department.
  • Report pertinent information regarding patient problems, personnel problems, progress re: projects and initiatives and any unusual occurrences to the Vice President.

Requirements:

  • Master's degree in Social Work and NJ Licensed Clinical Social Worker or Bachelor of Science in Nursing and Licensed as a Registered Nurse in the State of New Jersey with Master's degree in nursing or healthcare administration required.
  • Five years case management experience, and five years leadership experience in an acute care hospital setting required.
  • Effective communication and customer services skills.
  • High level of interpersonal and critical thinking skills to resolve complex problems which may require innovative solutions. Ability to negotiate in sensitive situations.
  • Knowledgeable in all aspects of case management, social work services, discharge planning/transitions in care and associated regulations/compliance requirements.
  • Case Manager and Case Management Administrator certifications desirable.

Salary Range: 119,283.00 - 190,791.00 USD

We offer competitive base rates that are determined by many factors, including job-related work experience, internal equity, and industry-specific market data. In addition to base salary, some positions may be eligible for clinical certification pay and shift differentials.

The salary range listed for exempt positions reflects full-time compensation and will be prorated based on employment status.
