Job Description

Position: Director, Intake & Transition Services

Function: Responsible for coordinating all agency intake and promoting client transitions to more independent living.

Reports to: Senior Director for Residential Programs

Location: Manhattan

Schedule: Mondays through Fridays (9:00am-5:00pm)

Tasks:

  • Reviews applications, sets up screenings, presents referrals to agency teams and recommends admissions in accordance with agency policy.
  • Aids and assists referral sources to other housing providers if applicants do not meet ACMH admission criteria.
  • Serves as the liaison to all agency programs in regards to admissions and internal transfers.
  • Provides consultation to program staff on opportunities for transition to more independent settings.
  • Assists with identifying eligible applicants for CORE & Respite programs as well as other non-residential ACMH services.
  • Interprets the agency’s programs and maintains liaison with hospitals, outpatient clinics and rehabilitation programs, shelters and drop-in centers for the homeless.
  • Maintains and disseminates internal vacancy memo to report on status of tours, admissions, internal transfers, discharges and census.
  • Maintains waitlist of applicants for programs.
  • Maintains client census reports and reports regularly to City and State agencies on admissions, discharges, vacant unit status, and leaves of absence.
  • Manages post-discharge survey and outcomes management system.
  • Facilitates quarterly transition meetings for all agency programs.
  • Facilitates Centralized Intake meetings.
  • Supervises Transition Specialists.
  • Recommends measures to evaluate and improve the quality of program services.
  • Attends in-service training as requested by supervisory personnel.
  • Serves as “Qualified Mental Health Specialist”.
  • Duties as assigned by supervisor.

Requirements

M.A. in the field of psychology or an analogous field or B.A. with significant experience in the field.

Salary: $85,000 - $90,000 commensurate with experience, plus generous benefits.
