About the Role:
As a Community Support Professional, you'll manage an assigned caseload, helping individuals achieve their daily living goals while fostering independence and community engagement. You'll play a vital role in supporting their social, emotional, and physical well-being, ensuring they lead fulfilling and valued lifestyles.
What You'll Do:
- Empower and Engage: Assist individuals in connecting with family and friends, and encourage active participation in community activities, such as shopping and leisure outings.
- Provide Daily Living Support: Help with basic activities of daily living, maintain safe and sanitary home conditions, and encourage positive personal hygiene and home maintenance.
- Champion Well-being: Provide behavior support, ensuring respectful interactions and helping individuals maintain appropriate social conduct.
- Manage Medical Needs: Schedule and accompany individuals to medical appointments, administer medications according to protocols, and proficiently use specialized medical equipment.
- Ensure Smooth Operations: Complete accurate documentation during each shift, and adhere to all organizational policies, procedures, and regulations.
- Continuous Learning: Participate in ongoing training sessions, including CPR/FA, Medication Administration, and more.
- Community Representation: Positively represent Siffrin and the individuals we serve in the community.
QualificationsWhat We're Looking For:
- Education: High School Diploma or GED required.
- Physical Ability: Must be able to lift at least 50 pounds, push wheelchairs, use Hoyer lifts, bend, twist, and pick up objects.
- Vehicle & Insurance: Must have a personal vehicle and maintain bodily injury liability insurance with agency-specified limits.
- Computer Skills: Basic computer proficiency (Word, Outlook, internet navigation).
- Background Check: Ability to meet agency policy requirements for criminal background and motor vehicle record checks.
- Flexibility: Ability to work an assigned schedule that may change based on individual needs, and willingness to be reassigned to other Siffrin locations if necessary.
