For nearly two decades, Consumer Direct Care Network Arizona (CDAZ) has been a trusted provider of home care services to Arizonans eligible for the Arizona Medicaid Personal Care Services Program. We specialize in offering personalized caregiving that supports individuals in maintaining their independence and quality of life within the comfort of their own homes. Through our dedicated services, we empower individuals to remain engaged in their communities while receiving the care they deserve.
As a Direct Care Worker, you will be at the forefront of the company working in a rewarding, challenging, and dynamic environment as the go-to person for our members! In this important role, you will be responsible for supporting individuals of all ages with disabilities to high-risk members in need of immediate assistance. You will provide care for those who choose to maintain a quality of life in their own homes and communities. Successful candidates could manage up to five members per day, give or take depending on the caseload and will travel to the member’s residence to provide in-home services. These services are typically directed by a state plan to adequately assist and provide outstanding and personalized day-to-day care for each member.

Founded in 1990, Consumer Direct Care Network (CDCN) assists people who self-direct their in-home personal care. Self-direction empowers people to manage their own services and supports, so they have more choice and control over how their care is delivered.
CDCN provides quality care and support to families and people of all ages, including older adults, children, and individuals with physical, intellectual, or developmental disabilities and veterans, so they can remain safe, healthy, and independent in their homes and communities.
