Job Description

AltaMed offers mentoring services for youth as a supplement to educational and occupational skills training available through our existing Workforce Development program. Technology skills training, career exploration, and leadership development are all benefits for the thousands who have formally gone through our Workforce Development program.

Workforce Development coordinates of all of AltaMed’s internships, including AmeriCorps and clinical placement. More broadly, though, it exists to promote the sustainable growth of AltaMed’s workforce, one day seeking to supply 10-20 percent of our employees, and aims to ensure that AltaMed is hiring from within the communities we serve with greater retention in mind.

Job Overview

The Digital Health Navigator (DHN) is a patient-facing guide who helps AltaMed patients and their families successfully understand how to download and use digital tools that facilitate an easier approach to redetermination. The DHN delivers patient education, hands-on digital onboarding, technical assistance, and ongoing follow-up to ensure patients understand the renewal process, maintain their Medi-Cal coverage, and feel confident using AltaMed's digital services. The role also promotes adoption and utilization of AltaMed's digital health platforms, identifies and addresses barriers to digital access, and tracks the learning so we have a better understanding of our patient population with respect to digital tools. The DHN manages an assigned panel of patients, provides personalized outreach throughout the renewal process, and may facilitate group education sessions for patients and health plan members with limited digital literacy. The DHN remains an advocate and supportive liaison with these patients and families beginning with the pilot in fall of CY 26, and during the first two cycles of renewal in CY 27.

Minimum Requirements

  • High school diploma or equivalent required.
  • Associate or bachelor’s degree in healthcare, Social Work, Public Health, Communications, or a related field preferred.
  • Demonstrated experience in customer service, sales, engaging challenging populations, community health, patient navigation, outreach, or public-facing technical support.
  • A basic understanding and experience assisting patients with Medi-Cal, Covered California, or other state or county health insurance programs is strongly preferred, but training and support will be provided.
  • Bilingual in English and Spanish required. In addition, demonstrated ability to work with patients whose primary language is one of our top needs (e.g., Vietnamese, Mandarin) is also helpful.
  • Proficiency using screen sharing, conducting webinars, and other methods to engage and educate patients on digital tools.
  • Experience working with underserved or diverse populations preferred.

The Workforce Development staff appreciates your interest in this opportunity. Should you be moved forward in the selection process, either the hiring manager or someone on the Workforce Development staff will contact you with next steps. We highly encourage you to continue to check-in for additional internship opportunities.

About AltaMed Health Services

AltaMed is one of the nation’s largest community health networks. For more than 55 years, we have provided care to patients in the diverse neighborhoods of Los Angeles and Orange counties. For health care professionals dedicated to their patients, communities, and careers, AltaMed offers an opportunity to pursue professional goals with pride and distinction.

As a non-profit organization, we focus on the needs of our patients and the well-being of our communities. Our commitment to community health pushes us to look beyond our offices and clinics and see not just patients, but new possibilities for community wellness. Many of our employees joined us because their families were AltaMed patients and saw firsthand that patients are at the center of all we do.

This approach supports our ambitious goals. We are continually accredited by The Joint Commission (JCAHO), which ensures quality standards in U.S. medical facilities. We were the first community health network in the nation designated as a Primary Care Medical Home (PCMH). We are extremely proud of our patient satisfaction achievements, including our highest scores during our greatest period of growth. We have expanded to 40+ locations in Southern California, offering primary medical, dental, and senior care. We are active in community outreach and education and staffed by dedicated professionals committed to patient care.

Always looking for ways to serve more people, AltaMed leads the industry in bringing new individuals into our nation’s health care system as the top enroller for Covered California. As we continue to grow, we remain at the forefront of evolving health care systems, providing comprehensive care throughout patients’ lives. We will continue to build on our reputation as a rewarding long-term choice for both patients and professionals who believe quality health care is everyone’s right.
