Oseca Health is seeking a passionate and experienced Dietitian / Diabetes Educator to join our team in Busselton. This role plays a critical part in improving health outcomes for individuals living with diabetes by delivering evidence-based education, care coordination, and self-management support.

Working collaboratively within the Integrated Chronic Disease Care Program, you will provide person-centred diabetes education, support multidisciplinary care planning, and partner with General Practitioners and other healthcare providers to ensure coordinated and effective diabetes management. Key Responsibilities

• Provide individualised nutrition and dietetic services to support clients with weight management, healthy eating, chronic disease prevention, and overall wellbeing.
• Deliver diabetes education and self-management support in line with ADEA Best Practice Guidelines and relevant professional standards (where accredited).
• Conduct comprehensive nutritional and health assessments and develop tailored care plans that align with client needs, goals, and preferences.
• Support clients to achieve sustainable lifestyle changes through evidence-based nutrition counselling, behaviour change strategies, and health coaching.
• Work collaboratively with GPs, nurses, allied health professionals, and community services to deliver coordinated, person-centred care.
• Monitor client progress, review outcomes, and adapt interventions to optimise health and wellbeing.
• Facilitate group education sessions, workshops, and health promotion activities focused on nutrition, diabetes management, healthy lifestyles, and disease prevention.
• Promote preventative health initiatives and support clients to improve self-management skills and long-term health outcomes.
• Maintain accurate clinical records, prepare reports, and communicate effectively with referring practitioners and members of the healthcare team.
• Participate in quality improvement, program evaluation, and data collection activities to support service excellence and organisational objectives.
• Contribute positively to a multidisciplinary team environment while complying with professional, organisational, and Work Health and Safety requirements.

To be considered for this role, you must meet the following criteria:

• Tertiary qualification in Dietetics or a recognised qualification in Diabetes Education.
• Current registration and/or accreditation with the relevant professional body.
• Demonstrated experience delivering diabetes education and supporting chronic disease management.
• Strong understanding of diabetes best-practice guidelines and patient-centred care principles.
• Ability to develop effective care plans and support self-management strategies.
• Excellent interpersonal, communication, and stakeholder engagement skills.
• Experience working collaboratively within multidisciplinary healthcare teams.
• Strong clinical record-keeping, reporting, and organisational skills.
• Commitment to continuous professional development and quality improvement.
