Provides comprehensive medical nutrition care for patients through nutrition assessment, individualized care planning, nutrition education, and evidence-based interventions. Collaborates with the interdisciplinary care team to support patients' nutritional needs and promote positive health outcomes.
Essential Functions
-Assess patients' nutritional needs using physician orders, clinical data, interdisciplinary care plans, and evidence-based standards to develop and implement individualized nutrition care plans that address age-specific, psychosocial, and clinical needs.
-Monitor patient progress and adjust nutrition interventions as appropriate to support optimal outcomes.
-Document nutrition assessments, care plans, recommendations, patient progress, consultations, discharge planning, and other nutrition-related clinical activities in accordance with organizational and regulatory standards.
-Provide nutrition counseling and education to patients and their families on therapeutic diets and healthy nutrition practices, developing educational resources as needed.
-Collaborate with food and nutrition services to ensure appropriate meals, supplements, and nutrition support are provided, and review menu selections to promote adherence to prescribed diets.
