Job Description

Evaluates, assesses, and plans nutrition care to meet patient nutritional needs.

Responsibilities

  • Assesses patient nutritional status using documented medical information and established nutritional standards.
  • Counsels patients and/or families by identifying nutrition-related problems. Develops plans of instruction and nutritional care, and monitoring results.
  • Identifies inpatients at nutritional risk and documents recommendations.
  • Diagnose and develop intervention for patients with malnutrition
  • Participates in physicians' rounds, discharge planning rounds, and conferences with nutritional implications.
  • Conducts education programs for patients, community groups, and staff.
  • Precept and mentor new staff or existing staff in clinical specialty area.
  • Develop therapeutic relationships with multiple subspecialty care team members and providers that enables planning and participating across a broader spectrum patient care.
  • Cross-covers a broader spectrum of patient care areas and subspecialties as needed.
  • Performs other duties as required or assigned.
  • Qualifications

    • Bachelor’s Degree from an accredited program in nutritional sciences (prior to 2024).
    • Master’s Degree from an accredited program in nutritional sciences (2024 and after).
    • Minimum 2 years’ related experience preferred.

    Required Licensure/Certifications

    - Current NH Dietitian license required. May require additonal license if providing Telehealth services.- Current Registration by the Commission on Dietetic Registration.
