Job Description

By The Bay Health, a non-profit established in 1975, set the standard for hospice in the U.S. by emphasizing the role of the patient in making important medical decisions. Our spectrum of home-based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care and Pediatric Care. Our team approach strives to address practical, social, emotional and spiritual aspects of care, with the goal to maximize quality of life for our patients, caregivers and families.

The Dietitian provides dietary instruction, consultation, and counseling directly to patients and/or to transdisciplinary teams to maximize the comfort and nutritional status of Hospice patients.

Schedule:

Per diem, must be available at least 6 shifts per month; 8:30am - 5:00pm

Marin, Sonoma & San Francisco Counties

By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.

Essential Duties & Responsibilities include (but are not limited to):

  • Participate in the development of the patient/family plan of care.
  • Make home visits, as requested by the transdisciplinary team, for nutritional assessment, patient/ family counseling, diet instruction, and identification of factors relating to variances of weight and/or size as they pertain to the dietary care of the patient.
  • Develop a nutritional plan, including identification of factors relating to variances of weight and/or size, based on patient evaluation and on the goals recommended by the transdisciplinary team and agreed to by the patient.
  • Provide consultation to agency professional staff regarding patient nutrition plans and problems and attend transdisciplinary team conference on a pre-determined schedule and as needed based upon patient/family plan of care needs.
  • Provide instruction in nutritional care of the patient to other members of the health care team including family members and attendants

Education/Experience

  • Bachelor’s (B.A.) degree in Nutrition or related field from an accredited four-year college or university.
  • Minimum of two years of experience providing dietetic consultation preferably in an acute care, home health or hospice setting.

Certificates, Licenses, Registrations:

  • Registered by the Commission on Dietetic Registration of the Academy of Nutrition and Dietetics.

Pay Range:
$40.00 - $50.00 hourly
