Job Description

Sun River Health provides the highest quality of comprehensive primary, preventative and behavioral health services to all who seek it, regardless of insurance status and ability to pay, especially for the underserved and vulnerable. Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking the following healthcare professional to join our team, supporting our Monticello, Spring Valley, Haverstraw and Peekskill health centers. We currently have the need for a Nutritionist at our Amenia and Peekskill locations.

The Dietitian will perform individual counseling of patients referred by the medical personnel in all sites of Hudson River Healthcare. Performs job duties in compliance with systems and job specific standards.

Essential Job Duties and Responsibilities:

  • Provides nutrition counseling to patients referred by clinicians
  • Performs initial nutrition history on each patient
  • Develops and implements a care plan based on the assessment of nutritional and health needs
  • Provides instructions on a normal or special diet
  • Schedules follow-up appointments and evaluates the reaching of goals
  • Provides nutrition counseling for diabetic patients as part of diabetic care plan
  • Provides nutritional education materials to patients to support counseling efforts
  • Makes referrals to other health care providers and community services as appropriate
  • Counsels individuals and families on nutrition and diet based on the particular needs of the individual or family as determined by their nutritional and health risk
  • Attends staff meetings and in-service training sessions at both local and state levels
  • Provides consultation into the development of program policies and procedures
  • Provides direction and executes health education programs as needed
  • Must demonstrate knowledge and skill necessary to provide care appropriate to the age of the patients served
  • Must demonstrate knowledge of the principles of grown and development over the life span

Minimum Education Requirement Bachelor’s Degree

Related Work Experience 2-4 years

Preferred Education Requirement: Master’s Degree

Preferred Work Experience 5-8 years

Required Licensure: RD/ RDN and CDN with a valid CPR card

Specialized Skills & Knowledge:

  • Ability to communicate in English, both orally and in writing
  • Ability to operate a motor vehicle and travel within the HRHC service site area
  • Knowledge of nutrition principles as well as knowledge of nutrition during pregnancy and lactation, as well as infant and child feeding practices
  • Knowledge of the geographic community serviced and resources
  • Competence in nutrition assessment counseling
  • Ability to collaborate with other member of Hudson River HealthCare staff in order to maintain high quality standards and achieve client satisfaction

Salary Range: $23 - $27 per hour

About Sun River Health

In the early 1970s, four African American women in Peekskill, New York came together to address the lack of affordable, quality health care in their community. With money from a small federal grant, the first Sun River Health site was opened in 1975.

That first health center has since grown to become an FQHC (federally qualified health center) system with over 40 locations, serving more than 250,000 patients throughout the Hudson Valley, New York City, and Long Island. Our exceptional primary care practitioners, specialists, and support staff have made us a destination for affordable, high-quality care.

In December 2018, Brightpoint Health, an FQHC network offering integrated medical, behavioral, and social support services in New York City, merged with Hudson River Health Care. Together with our subsidiary organizations, we became Sun River Health in December 2020.
