GENERAL DUTIES
Clinical Duties
1. Demonstrate an advanced knowledge of nutrition and counseling practices.
2. Complete individual personalized nutritional assessments and coordinate on-going nutritional care in recovery
3. Provide nutritional counseling education for clients and/or their families
4. Participate in care coordination and treatment planning
5. Assist in meal planning and preparation for client meetings
6. Lead and facilitate group nutrition classes on assigned topics
7. Communicate compassionately, effectively and enthusiastically with clients and/or families
8. Develop and maintain nutrition education materials and clinical procedures up to date with current research and practice
9. Must be able to demonstrate basic computer skills with a working knowledge of various software programs and electronic medical record charting
10. Completes assessments, screens, and follow-ups in a through and comprehensive manner, in accordance with the departmental and professional standards of care, including diet/intake histories, lab values, patient disease knowledge, drug-nutrient interactions and recommendations.
11. Educates patients in a through and effective manner.
12. Communicates and monitors implementation of the nutritional care plan with multidisciplinary team members.
13. Assists with discharge planning on inpatients.
14. Participates in health care team conferences and rounds assigned.
15. Maintains age and disease-specific clinical knowledge and skills.
Dietary Duties:
1. Assists diet office with menu options if needed, specifically for patients with special diets and food allergies.
2. Communicates with the Dietary staff regarding pertinent patient care, departmental performance and organizational issues as needed.
3. Initiates diet and laboratory orders within scope
Administrative:
1. Maintains up-to-date patient education materials for the assigned areas. Updates and/or develops education materials as needed.
2. Serves on hospital committees as needed and requested.
3. Actively participates in QAPI projects as requested.
4. Participate in collaboration of care and marketing to area providers
5. Utilize technology effectively and efficiently for scheduling, communications, and record keeping
6. Maintains accurate records and completes documentation in a timely manner
7. Participate in social media and web campaigns, advertising, and promotion.
Patient Care and Education:
1. Using evidence-based weight-inclusive practices, conducts nutrition assessments, interventions, and follow-up in a patient-centered manner for patients referred by area providers
2. Collaborates with a team of providers, dietitians and Psychologists
3. Participate in case conferences with interdisciplinary teams for patient care (mental health, primary care, etc)
4. Provide support for diabetes and nutrition group education programs, and in some cases develop, maintain, and conduct group education programs
5. Utilize motivational interviewing (MI) skills to support patients in a patient-centered manner.
