🌟 We’re Hiring: Diabetes Practitioner

Field based with travel around Black Country

Permanent, zero hour contract

ÂŁ12.48 per hour (admin rate), ÂŁ15.79 (1-1 session) plus mileage paid

Hours: Tuesday 12-8, Wednesday 9-5 ,Friday 9-5

As a Diabetes Practitioner, you will support individuals enrolled on our Type 2 Diabetes Remission Programme, helping them reduce their blood sugar levels and work towards improved long-term health outcomes over the course of a one-year Programme.

Through personalised coaching sessions delivered either in person, by telephone, or through digital platforms, you will empower and support service users to make sustainable lifestyle changes. You will help them set realistic goals, maintain motivation, and remain engaged throughout their health journey. By providing ongoing guidance, you will support individuals in overcoming barriers, building confidence, and sustaining progress towards remission and improved overall wellbeing.

  • Deliver personalised one-to-one health coaching sessions to service users.

  • Support participants to make sustainable Behavioural and lifestyle changes.

  • Help individuals set and achieve realistic health and wellbeing goals.

  • Monitor progress and encourage continued engagement with the Programme.

  • Build trusting relationships through a compassionate and person-centred approach.

  • Work collaboratively with colleagues to deliver high-quality outcomes for service users.

🎯 What You’ll Bring

  • Minimum of undergrad Degree in Nutrition or Health Related Science (Sports/Exercise Science/Health Psychology OR experience in personal training with a level 2/3 PT qualification

  • Phone based experience & technologically adept with Microsoft packages & systems.

📚 Experience and Qualifications We’re Looking For

  • Relevant health and wellbeing qualification such as (but not limited to) an undergraduate degree, diploma, or equivalent in Nutrition, Dietetics, Health Psychology, Sport Science and/or a CIMSPA fitness qualification at level 3 or above
