Job Description

Under general supervision, assesses, plans, implements and evaluates the diabetes in-patient education program through FPI at UMMC and UMMC-MTC. Plans and conducts continuing education programs in diabetes for patients, staff, and community partners. Functions as a resource for patients and other healthcare professionals within and outside the Medical Center.

Support the day-to-day conduct of the MDMOM project at UMMC; coordinate with the Telehealth Services at UMMC regarding the telemedicine platform and technology use; respond to consult requests from Network associate health facilities; schedule consults; facilitate consults and troubleshoot problems, including technology-related problems; collect Network assessment metrics and MDMOM reporting data; submit data reports to MDMOM as agreed upon with all Network participants; contribute to manuscript development.

ESSENTIAL FUNCTIONS

  • Utilizes a patient and family-centered approach
  • Collaborates closely with other team members, including providers, social workers, care managers, pharmacists, hospital dieticians, and community partners.
  • Empowers patients and caregivers to actively participate in their care.
  • Provides a clear and feasible discharge management plan based on clinical needs, patient preferences, goals, and abilities.
  • Ensures that a follow-up appointment for diabetes care is scheduled prior to discharge.
  • Establishes partnerships with community-based providers and services to address health and social needs.
  • Communicates the status of diabetes self-management education, plan of care, and medication reconciliation to next provider and/or site of care as needed.
  • Identifies and mitigates risk factors for readmission.
  • Places peripheral IV (intravenous) lines and obtains blood draws if that is needed.

EDUCATION and/or EXPERIENCE

  • Graduation from an accredited dietician/nursing education program approved by the Board of Dietetic Practice/Board of Nursing or found by the respective Board to be substantially equivalent to the programs in Maryland. Current licensure as a Registered Dietician/Nurse in the state of Maryland is required.
  • Must hold licensure as a dietitian or nurse in the state of Maryland, or eligible to practice as outlined through the Maryland Board of Dietetics/Nursing.
  • Certification as a Diabetic Educator (CDE) is preferred; or must obtain CDE certification within one year of service.
  • Knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient’s requirements relative to his or her age-specific needs and to provide the care needed as described in the unit/area/department policies and procedures.
  • Highly effective verbal and written communication skills are necessary in order to educate patients and their families and work with medical, nursing and management staff.
  • Demonstrated ability in public speaking preferred.

Total Rewards
The referenced base salary range represents the low and high end of University of Maryland’s Faculty Physician’s Inc. salary range for this position.Some candidates will not be eligible for the upper end of the salary range. Exact salary will ultimately depend on multiple factors, which may include the successful candidate's geographic location, skills, work experience, market conditions, internal equity, responsibility factor and span of control, education/training and other qualifications.
