Days (United States of America)Act as coordinator of the Pediatric and Adult Diabetes Program, providing self-management education and resources for patients across the continuum, guiding them and their caregivers toward a quality outcome.

Major Job Functions

The following is a summary of the major essential functions of this job. The incumbent may perform other duties, both major and minor, that are not mentioned below. In addition, specific functions may change from time to time:

  • Function as a part of the leadership and coordination team (Medical Director, Physicians) of the Pediatric and Adult Diabetes Program

  • Collaborate with Information Systems staff for the maintenance of the diabetes data collection system

  • Collaborate with diabetes team and outside providers in the development and implementation of individualized educational plan of care for each patient

  • Assess health status, learning needs, and self-management process of referred patients with initial class presentation and with follow-up contacts per program guidelines

  • Facilitate development and use of protocols and plans patient care in cooperation with the diabetes center team

  • Discuss plan of care with the patient and family at the direction and discretion of physicians

  • Monitor self-management processes based on assessment of physical, psychological, and spiritual responses and financial needs of patients, families, or significant others and alerts the health care team to actual and potential health related problems, and unusual occurrences

  • Evaluate care and coordinates interventions of referred patients in conjunction with all members of the healthcare team

  • Develop strong supportive relationships with referred patients, hospital departmental personnel, physicians, other disciplines and community-based agencies and community physicians

  • Maintain knowledge of and participate in CQI and QAI activities

  • Conduct outcomes evaluations including effectiveness of follow-up procedures and make results available to physicians and the multidisciplinary team

  • Retrieve and report outcomes data indicators that show the impact of diabetes education on patient health status, and reports results to the healthcare team and the collaborative practice group

  • Ensure data is collected and reported to the ADA for continued certification of the diabetes program

  • Conducts periodic diabetes education programs to staff and community groups in coordination with Education Department as required

  • Responsible for raising community awareness about diabetes education through public service programs in coordination with Marketing Division as appropriate

  • Guide and educate assigned interns and student dietitians during their rotation with the Diabetes Program

  • Ensure compliance with all local, state, and federal requirements for documentation and registration of outpatients per CFVHS corporate compliance and other policies

  • Maintain and adhere to policies and procedures that are consistent and in compliance with the American Diabetes Association’s nationally recognized program requirements

  • Other duties as assigned

Minimum Qualifications

The following qualifications, or equivalents, are the minimum requirements necessary to perform the essential functions of this job:

Education and Formal Training

  • Bachelor’s degree in Health Sciences or related healthcare field required

  • Current healthcare licensure (in field of study) in North Carolina required

  • Current Certified Diabetes Educator (CDE) required

  • Registered Dietitian preferred

Work Experience

  • 3 years of current hospital experience in clinical area of responsibility required

  • 1 year management experience preferred

  • 1 year case management experience preferred

Knowledge, Skills, and Abilities Required

  • Maintain current knowledge and skills in diabetes self-management and education concepts, practices, and techniques and disseminates information to the health care team through appropriate forums

  • Positive interpersonal communications skills

  • Outstanding verbal and written skills

  • Demonstrate effective organization, prioritization, and leadership skills

  • Knowledgeable of the age specific physiological, psychosocial, developmental, and spiritual needs of the patient population typically served

Physical Requirements

  • Ability to stand and walk for extended periods

  • May be required to bend and lift

  • Ability to access all areas of the hospital including detached buildings

  • Excellent speech, auditory and visual acuity for communication of patient needs and observation of patients

  • Position may involve skin, eye, mucous membrane, or parenteral contact with blood or other potentially infectious materials
