Job Location Crestview Dental - Crestview , FL 32539 Education Level Professional License Required



Provide the full scope of dental health care services which fall under his/her field of training, including but not limited to diagnosis, treatment, coordination of care and maintenance to patients of all ages.

SCHEDULE/HOURS:

  • Full Time
  • Monday - Friday
  • Typically 8:00 am - 5:00 pm
  • Rarely weekends



Qualifications

EDUCATION:

  1. Graduation with a D.M.D. OR D.D.S. degree from a Dental School accredited by the Commission on Accreditation of the American Dental Association (A.D.A.) preferred. Graduation from a school not accredited by the A.D.A. must have completed a two year supplemental dental education program at an A.D.A. accredited dental school.
  2. Licensed by the State of Florida to practice Dentistry.
  3. Current Cardiopulmonary Resuscitation Certificate (CPR)

North Florida Medical consists of 10 primary care and 3 dental locations, providing high quality care in service areas that were traditionally underserved. We are in Crestview, Quincy, Wewahitchka, Eastpoint, Tallahassee, Panacea, Madison, Perry, Mayo and Cross City. Most recently, we have added a mobile clinic, Health Force One. Our corporate offices support our clinics and are based in Tallahassee.

Our clinics are Federally Qualifing Health Centers and as such can offer a sliding fee program in addition to taking insurance (including Medicaid and Medicare). Our current slide fee program begins with a nominal fee as low as $10.
