We’renow seeking a Dementia Care Partnerto join our team, servicing clients across theHills area and Northern region (including Northern Beaches, Lane Cove, Ryde, Hornsby, Pennant Hills,Carlingfordand Castle Hill.
Your Opportunity

Part Time | Monday to Thursday | 30.4 hours per week | 18‑month contract
Hours:8:30am – 4:30pm or9:00am – 5:00pm
This is acommunity basedrole supporting people living with dementia and their families through education, guidance,advocacyand tailored support.
You’llwork closely with clients in their homes and in community settings, helping them navigate services, understanddementiaandmaintainquality of life.
In this role, you will:
Provideperson-centredsupport, advice and advocacy to clients living with dementia
Conduct assessments and develop care plans to support individual needs
Confident in presentingDementia Education withinAnglicareand theCommunity
Deliver education and psychosocial support to families and carers
Support clients to access services and navigate the aged care system
Monitor client progress and adapt support as needs change
Maintainaccuratedocumentation and meet program KPIs
Build strong relationships with families,cliniciansand community networks

You’reempathetic,confidentand skilled at working with people experiencing complex challenges. You bring both compassion and practicalproblem-solvingto your work.
You will also bring:
Experience in aged care,communityordementia relatedroles
Strong communicationskills and the ability to build trust with clients and families
Understanding of dementia and its impact on individuals and carers
Experience with assessments, careplanningor case management
Relevant qualification in Community Services, Dementia Care or similar (essential)
Current driver’s licence and access to a vehicle

The Royal Children's Hospital (RCH) has been providing outstanding care for Victoria's children and their families for over 147 years.
We are the major specialist paediatric hospital in Victoria and our care extends to children from Tasmania, southern New South Wales and other states around Australia and overseas.

We are the designated state-wide major trauma centre for paediatrics in Victoria and a Nationally Funded Centre for cardiac and liver transplantation.
