Job Description

Are you ready to make a real impact in the lives of individuals with disabilities? Do you have a passion for helping others reach their fullest potential? If so, New Avenues to Independence is looking for someone just like you to join our team as a Day Services Coordinator!!

The Days Services Coordinator assists individuals with disabilities to participate fully in meaningful activities in their communities. The Day Services Coordinator may work with individuals on a one-to-one basis or in small groups, and serves as New Avenues point of contact for one or more community partners. In addition to providing services and supports to assist in achieving individualized goals and outcomes, the Day Services Coordinator is responsible for documenting service provision in a timely manner. This position will also be responsible for maintaining regular communication with each person served as well as team members regarding progress, outcomes, and unusual incident reports.

Requirements

Qualifications:

  1. Two years of experience in DD field.
  2. Must be at least 18 yrs. of age.
  3. High School Diploma or GED.
  4. Experience in day service and/or employment settings.
  5. High energy level with enthusiasm for connecting to the community.
  6. Must obtain and maintain CPR/FA, PBS, and Medication Administration certifications.
  7. Demonstrated computer skills including MS Office and G-Suite.
  8. Valid Ohio Driver’s License that meet agency standards.
  9. Must have reliable transportation to transport individuals as needed.
  10. Maintain auto insurance.

New Avenues has developed a continuum of services, meeting the changing needs of individuals with disabilities throughout the course of their lifetime. New Avenues’ programs are currently serving over 600 individuals, ages 3-100 years in Ashtabula, Cuyahoga, Geauga, Lake, and Summit counties, Ohio. The individuals served include those with disabilities and chronic health conditions such as developmental disabilities, mental illness, autism, cerebral palsy, brain injury, and Down syndrome.
