Grounded in trauma-informed care and cultural humility, Net Community Care works to stabilize families, support youth development, and ensure children can grow in safe, nurturing, and permanent homes. The organization is committed to strengthening communities through collaboration, accountability, and continuous learning.
SUMMARY:
The Aftercare Worker at Net Community Care, supports families who have successfully exited formal case-management services through DHS/CUA. This role focuses on reintegration planning, resource connection, and ongoing consultation to reduce the likelihood of re-entry into the child welfare system. The Aftercare Worker collaborates with case management teams, children, biological families, and external partners to ensure families are equipped with the tools and support needed for long-term stability.
