Job Description

POSITION SUMMARY
Reports To: Program Director
Trainings: Annual, CPI, HSFAC required within 90 days from date of hire or transfer.
Position Summary: Respond to crisis in community within 24 hours. De-escalate crisis and stabilize child and family unit. This position provides assessment, treatment planning and comprehensive case management.
Duties and Responsibilities:
Responds on site to the 1-800 state triaged crisis call within one hour.
Utilizes the DHS assessment tool for identification of risk.
Provides crisis stabilization services geared toward preserving the community placement up to a maximum 72 hour period, if assessment indicates no immediate danger to self and/or others.
Works with the screening center to support on site (community based) screening, when indicated.
Coordinates the inpatient admission, if indicated and according to established affiliation agreement.
Links the child/family to community based services, when indicated and crisis stabilization is effected, according to established agreements.
Develops the Individual Crisis plan for a period not to exceed eight weeks if further stabilization is indicated.
Provides relevant feedback to the program coordinator re: provider network access and response to service authorization.
Documents all services according to established standards.
Provides timely relevant/required data to the Project Director as directed.
Documentation must be completed within one (1) calendar day of service delivery
Minimal Qualifications:
Master's Degree in a behavioral health or related human services field.
Bachelor's Degree in a behavioral health or related human services field and have a minimum of one year related field work experience
Preferred:
Bachelor's Degree with three years' experience in working with children/families, and/or behavioral health.
Master's Degree with one year experience with children/families and/or behavioral health.

Licensure: Valid driver's license
Certifications:
Equipment Use: Computer, copier, facsimile, and shredder. Driving record to pass check and ability to drive in day/night and reasonable weather conditions.
