What will your day look like?

Our highly regarded onboarding program will offer you direct mentorship with experienced counselors, individualized training, and a team of coworkers who want to ensure your success here at Horizon! Some of the day-to-day responsibilities you will have include:

  • Complete 4-hour SBIRT training course.
  • Work as part of the school counseling team at SBIRT partnership school locations.
  • Become familiar with use of CRAFFT 2.1+N, PHQ-A, and PHQ-9.
  • Implement crisis and lethality clinical protocols as clinically indicated including Safety Plans and the CSSRS.
  • Become familiar with cannabis and nicotine prevention and intervention strategies
  • Become familiar with all consent and referral forms to be used with population of focus.
  • Obtain parental/legal guardian consent for screenings.
  • Screen adolescents and young adults (clinician-administered unless otherwise indicated) for substance use disorders (SUDs) using screenings exactly as they are without modifications.
  • Conduct Brief Interventions, per screening results, for adolescents and young adults.
  • Provide validated screening tools in the language of the student’s preference and enlist support from language interpretation services to communicate effectively with students and families when needed.
  • Work closely with families and SBIRT Family Peer when an adolescent/young adult screens positive for SUDs to ensure a full profile of behavior is carefully documented and reported to all members of the treatment team.
  • Utilize tools included in SBIRT psychoeducation package to distribute to adolescents and young adults.
  • Initiate Referral to Treatment in allotted timeframe if screening results and Brief Intervention outcomes warrant it and conduct warm “hand off’ whenever possible.
  • Work with Project Director to perform referrals and ensure linkage within appropriate timeframe.
  • Ensure timely documentation of SBIRT outcomes to referral sources and complete status updates as requested.
  • Work with Project Director to ensure follow-up is conducted in regular intervals following referral.
  • Document and submit all patient data in appropriate systems and to appropriate members of the SBIRT team for reporting and evaluation use.
  • Maintain complete confidentiality of all patient information/data throughout 5-year program.
  • Utilize EBP techniques with full fidelity to models.
  • Collaborate with all members of SBIRT multi-disciplinary team as well as other providers to ensure a full continuum of care for all SAFER
  • Attend all SBIRT meetings.
  • Assist with clinical and operational implementation of the Tickit Health classroom-based wellness screening tool.
  • Participate in community engagement and marketing events as requested.
  • Participate in all repeat trainings necessary through course of program.

What has prepared you for this opportunity?

In addition to all of the all-nighters and cramming for exams…

  • CASAC-T (Credentialed Alcohol and Substance Abuse Counselor) Certification or higher plus a Bachelor’s Degree

OR

  • Completed Master’s degree in a program that will grant licensure in the state of New York
  • Prior experience working in the MH/SUD field preferred.

Location:

This position is located at 699 Hertel Ave, Buffalo NY. You must be willing to travel to schools in WNY as needed.

Position Hours:

This is a full-time position from Monday – Thursday 8:00am – 5:15pm & Friday 8:00am – 1:00pm.

Compensation:

Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive. The range of pay for this position is $48,750-$55,000. Starting pay may be impacted based on experience and education. Compensation adjustments are evaluated annually.

Physical demands associated with this position include:

  • Sitting for extended periods, alternating between standing and sitting
  • Mobility required with repetitive wrist, hand, and finger movements
  • Handling light duties, occasionally lifting objects up to 20 pounds
  • Travel to other worksites
