COTA - Skilled Nursing Facility | Travel or Permanent

Blue United Sourcing has two openings for experienced Certified Occupational Therapy Assistants (COTAs) at a skilled nursing facility in San Jose, CA. Both positions are available for an ASAP start and open to travel, permanent, or travel-to-perm candidates. Silicon Valley is calling — two spots available.

Details of the role:

  • Anticipated start date: Flexible
  • Salary range: Up to $1,800 weekly
  • Anticipated duration of contract: 13 weeks
  • Contract type: Travel, Permanent, or Travel-to-Perm

Clinical Duties:

  • Assist with evaluations and implement OT treatment plans.
  • Provide therapy to improve residents’ ADLs and functional mobility.
  • Support interventions for orthopedic, neurological, and post-acute conditions.
  • Teach adaptive techniques and proper use of assistive devices.
  • Collaborate with OTs, nursing, and rehab teams on care goals.
  • Document therapy sessions, progress, and daily notes accurately.
  • Monitor resident responses and report outcomes to supervising OT.

Requirements:

  • Current COTA license in the state of California.
  • Associate’s or higher degree in Occupational Therapy Assistant from an accredited program.
  • Ability to follow OT treatment plans and assist with interventions.
  • Strong communication and teamwork skills for interdisciplinary collaboration.
  • Accurate documentation and time management skills.
  • Commitment to following facility policies and state regulations.
