Position Title: Ongoing Coordinator, Woodland Park
Department: Case Management (CMA)
Reports To: CMA Program Supervisor
FLSA Status: Non-Exempt
Starting Pay Range: $22.32 - $23.16

Ongoing Coordinators at The Resource Exchange (TRE) provide community-based case management services to individuals and families receiving long-term services and supports. This role primarily serves members living in Woodland Park and Bailey, with occasional support to surrounding areas based on member needs, service availability, and continuity of care.
Ongoing Coordinators work within a collaborative team model, sharing responsibility for coverage, compliance, and service quality. The position operates in a highly regulated Medicaid environment, requiring strong documentation practices, working knowledge of HCPF and waiver requirements, and a commitment to person-centered, ethical service delivery for vulnerable populations.

Essential Duties & Responsibilities
Case Management & Service Coordination
- Serve as the primary ongoing point of contact for individuals receiving services in Woodland Park, Bailey, and surrounding Park and Teller County communities
- Provide occasional case coverage or support in nearby surrounding areas based on member needs
- Develop, implement, monitor, and update service plans in compliance with HCPF, LTC, HCBS, and waiver requirements
- Support individuals and families in navigating Medicaid services while promoting dignity, choice, and self-direction
- Build and maintain strong relationships with providers, community partners, and interdisciplinary teams
Documentation & Compliance
- Accurately document client interactions, eligibility determinations, and service activities in the Case Management System (CMS) and required databases, including BUS, CCMS, and BILI
- Complete Per-Member Per-Month (PMPM) activity reports in alignment with DDD rules, regulations, and quality standards
- Maintain up-to-date knowledge of Developmental Disability Determination (DDD) processes, Long-Term Care Medicaid eligibility, and TRE billing methodologies
- Conduct detailed research and maintain trackers related to financial eligibility, service utilization, and compliance
- Correspond with external agencies to update applicant and member information and manage referrals
- Maintain organized, audit-ready records in compliance with HIPAA and state and federal regulations
Customer Service & Communication
- Provide excellent customer service by responding promptly and professionally to emails and phone calls
- Communicate effectively with individuals served, families, providers, supervisors, and internal teams
- Ensure all communication follows confidentiality, privacy, and HIPAA guidelines
- Participate in departmental, agency-wide, and interdisciplinary meetings and trainings using tools such as Outlook, Office 365, Zoom, and other platforms
Team & Operational Support
- Collaborate with fellow Ongoing Coordinators to ensure continuity of services and shared accountability
- Provide coverage for other TRE roles as needed, including occasional after-hours availability
- Assist with special projects and administrative support tasks
- Maintain availability during customary TRE business hours, with flexibility to meet operational needs
- Perform driving duties related to the position while maintaining required licensure and insurance
Community-Based Responsibilities
- Conduct work in a variety of settings, including:
- - Private homes
- - Residential and community-based settings
- - Virtual and office environments
- Travel within Teller County, Park County, and occasionally surrounding areas
- Use personal transportation with mileage reimbursement in accordance with TRE policy

Qualifications
Minimum

Qualifications
- Bachelor’s degree in human behavioral sciences or an equivalent combination of education and relevant LTSS experience
- Minimum 5 years of experience working in Medicaid, intake, case management, or human services
- Familiarity with HCPF regulations, Colorado Medicaid waiver programs, and case management processes
- Strong interpersonal, organizational, documentation, and communication skills
- Demonstrated commitment to equity, inclusion, cultural responsiveness, and ethical service delivery
Preferred

Qualifications
- Experience in case management or intake coordination within a nonprofit or human services organization
- Working knowledge of Medicaid Waiver programs and HCPF rules
- Experience supporting individuals with disabilities and coordinating community-based services
Tools & Equipment Used
Computers, tablets, cloud-based systems, printers, telephones, and personal vehicles.
Working Environment & Physical Activities
- Work is performed in office, home, virtual, and community settings. Moderate physical activity is required, including walking, sitting, standing, and occasional lifting (up to 20 pounds).
- A valid Colorado driver’s license (unless exempt due to military status), reliable personal transportation, and minimum insurance coverage of $100,000/$300,000/$100,000 are required.
