Job Description

  • Works to help BPHC advance as an antiracist organization by meeting or exceeding the standards set by the BPHC Anti-Racism Policy.
  • Provides outreach, education, case management, advocacy, referral and support services to Boston residents with a particular focus on high-risk clients and second language populations.
  • Responsible for providing level of care assessment and referrals to substance abuse treatment throughout sites in Boston; identifies needs related to substance abuse treatment and makes appropriate referrals.
  • Assists clients with filling out assessment information regarding program participation and evaluation.
  • Facilitates linkage to other recovery support and substance abuse-related services.
  • Participate in community meetings and prepare presentations for same. Serves as liaison to assigned neighborhoods.
  • Participate in regularly scheduled staff meetings and training.
  • Responsible for the safe transportation of clients to various treatment programs in the Greater Boston Area.
  • Organizes, coordinates and manages office operations. Handles essential administrative tasks and provides program support when necessary.
  • Assists with coordination of meetings; develops draft agenda items, copies and mails agenda and materials; orders refreshments; attends meetings on behalf of dept when necessary.
  • Ensures an adequate amount of office supplies.
  • Assists with producing reports, including word processing and table/chart creation and with special MIS projects as needed.
  • Assists program management in report and grant preparation.
  • May delegate clerical and administrative duties and coordinate work efforts of others.
  • May be required to work on special projects and assist other departments, or act as a department liaison as needed.
  • Coordinates, monitors, develops and implements PAATHS programming to ensure effective operations and compliance with established standards and/or contracted goals & objectives.
  • Support clients through PAATHS care continuum when indicated.
  • Works collaboratively with members of the team and promotes the shared philosophy about respective, comprehensive and coordinated delivery of referral services.
  • Facilitate client access to recovery-oriented substance use disorder treatment, resources, and community recovery groups.
  • Monitor client progress in treatment and advocate for services. Assist client in navigating the healthcare system and social service systems. Accompany clients to appointment and court. Provide transportation as needed.
  • Provide support in treatment programs and emergency department settings as needed.
  • Work with treatment providers, hospital and BEST team staff as needed.
  • Facilitate communication between client and treatment providers and physicians.
  • Provide overdose prevention training and safety planning as needed.
  • Assist with client intake procedures.
  • Monitors completion of client's intake, psychosocial assessments and treatment plans.
  • Attend meetings as assigned. Serve as a critical team member on a multi-disciplinary team of providers serving high utilizers in the Mass/Cass neighborhood.
  • Represents BPHC in the community.
  • Coordinates activities which may include: fiscal monitoring; monitoring of funded programs or agencies to ensure compliance; report preparation and writing; and assisting with developing and distributing communications, brochures and educational materials. May supervise employees. Complete documentation and intake procedures. Maintain accurate and up-to-date records.
  • Working to help BPHC become an antiracist organization by meeting or exceeding the standards set by the BPHC Anti-Racism Policy.
  • Performs other duties as required.

Qualifications

  • BA/BS required OR 3 years of equivalent experience in public health program coordination, management and/or community organizing may be substituted for undergraduate degree. Licenses, certifications or program specific
  • experience may be required by grant or program needs.
  • Demonstrated experience or related requirements as established by the program may be needed.
  • Experience collaborating with clients and/or providers serving a variety of populations, including residents with cooccurring
  • mental health and addiction treatment needs.
  • Ability to prepare reports and to maintain accurate records.
  • Ability to write clearly and concisely.
  • Ability to understand, explain, apply the laws, rules, regulations, policies, procedures of program activities.
  • Ability to establish rapport with others and to maintain harmonious working relationships with others required.
  • Demonstrated knowledge & experience with MSOffice, MSWord/Excel, Windows and/or other software. Demonstrated knowledge of existing community resources and services in addiction treatment.
  • Demonstrated ability to establish effective communications and working relationships with community agencies and providers.
  • Ability to plan, develop, coordinate and evaluate health programs or services for the community.
  • Exceptional organizational skills with the ability to develop timelines and deadlines.
  • Demonstrated cultural competence with the diverse ethnic, cultural and socio-economic groups. Excellent interpersonal
  • skills and ability to communicate effectively.
