10 Gove Street – Taylor BuildingAn Outreach Coordinator works together with other staff in the CATCH Program to promote a positive atmosphere that enhances efficient workflow through good teamwork. The Outreach Coordinator performs a wide variety of clerical and reception activities that support the delivery of services to patients/staff of the Specialty Department. Will take direction from the CATCH Nurse Practitioner and will report to the Medical Director of the CATCH Program and the Administrative Director of Pediatrics.

Essential Duties & Responsibilities

  • Answers phones in a courteous and professional manner, using the four part greeting, which includes name, and department title. Screens and redirects calls appropriately.
  • Composes and types detailed and accurate messages using staff messaging system, and the Telephone Encounter. Calls patients as requested by a practice staff person. Manages both specialty appointment reminders and appointment recall system (wait list). Reviews emails received on a daily basis.
  • Uses computer to schedule, cancel, and confirm appointments; to requests records, to pull up and print information, and to communicate by e-mail. Maintains the practice files.
  • Interacts with patients or other staff in a professional manner, and recognizes the needs and concerns of the individual. Consistently provides excellent customer service and makes patients feel welcome. Maintains a positive attitude. Uses communication devices appropriately.
  • Reports safety and equipment problems, and malfunctioning or missing equipment to the supervisor.
  • Assists in expediting patient’s specialty needs by ensuring all the necessary documents and forms are available.
  • Is able to maintain the NP’s schedule for home visits. Is able to arrange appointments for needed vaccines including flu vaccine.
  • Completes forms, letters and other clinical paperwork as directed Community Resource Coordinator and NP.
  • Processes referrals as received with the appropriate specialist. Asks for additional information as needed to be accurate in making appointments.
  • Opens sorts and delivers mail. Sends, receives, sorts and delivers faxes. Ensures adequate paper supply for printers and faxes.
  • Identifies and verifies, and updates patient and demographic information.
  • Refers patients to member services, patient advocate and other departments. Provides information and directions to patients.
  • Participates in required departmental activities and meetings.
  • Follows established departmental policies and protocols.
  • Understands organizational structure of Specialty Department and Unit routing, and adheres to department policies and procedures (i.e., dress code, attendance, cell phone use, food at desk, etc.)
  • Maintain excellent punctuality and attendance.
  • Promote a sense of “team work” through demonstration of self-
  • direction and self-motivation. Solve problems independently or knows when to seek consultation.
  • Displays outstanding customer service skills when interacting with all EBNHC customers according to the ACE model.

CATCH PROGRAM Specific Responsibilities

Is able to access entitlement benefits

SSI: completing applications

  • Accompany parents to the interview appointments
  • Help interpret
  • Arrange transportation to meetings
  • Follow case through until first check arrives
  • Assist in financial needs such as helping with banking needs as necessary

Home Visits as needed

Assist the families in the home with the completion of forms for:

  • Insurance
  • SSI
  • Immigration
  • etc

Accompany and attend:

  • School evaluations (in home review for

comprehension)

  • Arrange transportation (school, appointments etc)
  • Update PT-1 on file
  • Remind patients of appointments/transport

Assist parents with:

  • Globe Santa, letters and applications
  • Summer Camp applications
  • Assist the Volunteer Department to match patients w/ correct volunteers (language/ social issues sensitive) help volunteers w/assisting @events

Coordinate: Annual Events:

  • Holiday Party
  • Summer picnic
  • N.E. Aquarium Vacation Program etc.

Perform all office tasks:

  • Faxing letters, referrals, prescriptions, letters of medical necessity, etc.
  • Organize and track guardianship paperwork
  • Applications for DDS,SSI, and other resources

Appointments:

  • Schedules specialist appts. and coordinates same day/or patient preferences
  • F/U on missed appts. And reschedule as needed
  • Coordinate school meetings
  • Schedule pre-op physicals, RHCM, sick visit appts
  • Monitor records to insure patients are up to date w/ P.E.s/Vaccines
  • Referrals
  • Schedule and check in check out home visits

Update Epic with specialist referral notes post appointment

EDUCATION: High School Diploma or G.E.D.

EXPERIENCE: At least six (6) months in customer service or an office

SKILLS/ABILITIES:

  • Demonstrated telephone skills (i.e. call screening, message taking and giving), in a professional manner.
  • Familiarity with use of personal computer in a Windows environment preferred.
  • Fluent in spanish
