Care Coordination | Patient Education | Home Health Referral Management
Part time 40 hours biweekly, rotating days M-F

The Admissions Coordinator for Home Health Care plays a key role in supporting that mission by serving as the primary liaison between Meritus Medical Center and partnering Home Health agencies. This role ensures patients receive timely, appropriate, and well‑coordinated post‑acute services that support recovery, safety, and continuity of care.
Under the supervision of the Clinical Manager, the Admissions Coordinator identifies potential referrals, assesses eligibility for home health services, educates patients and families, and collaborates closely with physicians and the care management team. This position actively supports discharge planning and evaluates the effectiveness of the referral process to promote high‑quality outcomes.
Serve as the primary liaison between Meritus Medical Center and Home Health agencies
Identify potential referrals and assess patient eligibility for home health services
Educate patients and families on home health care options, benefits, and expectations
Promote home health services to support safe transitions and improved outcomes
Coordinate the discharge planning process in collaboration with care management and physicians
Evaluate referral effectiveness and support continuous improvement in care transitions
Maintain accurate documentation and ensure compliance with regulatory and organizational standards
Bachelor of Science in Nursing (BSN) preferred
ASN required
Minimum of two (2) years of experience in a healthcare setting required
Experience in Home Health or community health preferred
Current Registered Nurse (RN) license in the State of Maryland OR
RN license from a Nurse Licensure Compact state with a Multi‑State Privilege designation
Note: A compact license designated as Single State does not meet requirements; a Maryland license must be obtained
Basic computer skills required
Proficiency in Microsoft Office (Word, Excel, PowerPoint, Outlook)
Meditech experience preferred
Experience with care management software preferred
Excellent communication and interpersonal skills
Public speaking and patient education experience preferred
Knowledge of Medicare coverage criteria preferred
