Continuing Care Retirement Community (CCRC) & Value-Based Care

Location: Flexible/Hybrid (Travel to communities as needed) based in Columbus OH
Job Type: Independent Contractor (1099)

We are seeking an experienced, collaborative physician leader to serve as our Contractor Medical Director This role provides clinical oversight for our Continuing Care Retirement Community (CCRC) and supports our growing value-based primary care initiatives. The Medical Director will partner closely with clinical leadership, Advanced Practice Providers (APPs), and operational teams to promote exceptional resident care, regulatory compliance, quality outcomes, and population health strategies.

Key Responsibilities

Medical Director / CCRC Oversight

  • Ensure physician availability 24 hours per day for emergency consultation and clinical support.
  • Provide clinical leadership across the Continuing Care Retirement Community (CCRC) to promote resident health, wellness, and quality outcomes.
  • Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
  • Review, develop, and provide guidance on clinical policies and procedures to ensure compliance with federal, state, and organizational standards.
  • Provide oversight of infection prevention and control programs.
  • Participate in regulatory survey preparation, readiness activities, and responses to survey findings.
  • Review clinical outcomes and quality indicators to identify opportunities for continuous improvement.
  • Support organizational participation in value-based care initiatives and population health programs.

Primary Care / Value-Based Care Oversight (At Your Door)

  • Perform physician visits, as appropriate, in support of value-based care initiatives.
  • Serve as the collaborating physician for Advanced Practice Providers (APPs).
  • Review clinical outcomes, quality indicators, and conduct periodic chart audits to ensure high-quality, evidence-based care.
  • Provide ongoing clinical oversight and mentorship to APPs.
  • Support care management programs and ensure alignment with population health and value-based care strategies.
  • Participate in business development discussions with prospective healthcare partners.
  • Provide clinical leadership through staff education, interdisciplinary collaboration, and stakeholder engagement.

Qualifications

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school.
  • Current, unrestricted medical license in the applicable state(s).
  • Board Certification in Family Medicine, Internal Medicine, Geriatrics, or a related specialty.
  • Experience serving as a Medical Director, collaborating physician, or physician leader in senior living, long-term care, post-acute care, or value-based care preferred.
  • Demonstrated knowledge of CMS regulations, infection prevention, quality improvement, and regulatory compliance.
  • Experience working with interdisciplinary teams and Advanced Practice Providers.
  • Excellent communication, leadership, and relationship-building skills.

Preferred Experience

  • Continuing Care Retirement Community (CCRC) experience.
  • Skilled Nursing Facility (SNF), assisted living, or senior housing experience.
  • Population health and value-based reimbursement models.
  • Regulatory survey readiness and QAPI leadership.
  • Experience supporting business growth and clinical program development.
