Overview

JOB SUMMARY:

The Consent Coordinator supports HealthDrive’s Care Management Program by traveling to skilled nursing and post-acute care facilities across company markets to educate residents and obtain informed program consent. This highly mobile, field-based position requires frequent air travel, overnight stays, and independent scheduling. The coordinator explains program services clearly and compassionately, ensuring each resident has the opportunity to make a voluntary, informed decision. Accurate documentation, daily activity reporting, and productive collaboration with facility and HealthDrive teams are essential to successful program enrollment.

ACCOUNTABILITY:

Responsible for independently executing assigned facility consent visits, accurately recording resident interactions and consent outcomes, and submitting complete daily activity reports. Owns assigned enrollment activity and timely escalation of barriers while respecting resident choice and all applicable consent and privacy requirements. This position has no direct reports or independent budget authority and manages approved travel expenses in accordance with company policy.

Responsibilities

  • Travel to assigned markets and facilities, including frequent flights and overnight stays, to conduct in-person consent visits. • Explain the Care Management Program to residents using clear, patient-centered language and appropriate pacing. • Answer questions consistently and respectfully, allowing additional time or repetition when needed. • Obtain and document voluntary informed consent using approved processes and forms; honor a resident’s decision to decline. • Coordinate visits with facility administrators, nursing teams, social workers, and HealthDrive colleagues. • Maintain accurate electronic records of resident contacts, consent outcomes, follow-up needs, and enrollment barriers. • Submit daily reports detailing activity, productivity, outcomes, and outstanding issues to Field Operations leadership. • Organize assigned schedules and travel plans to support timely market coverage and efficient use of resources.• Protect resident information and follow HIPAA, facility protocols, and company policies. • Escalate consent questions, concerns, or operational barriers to appropriate leadership promptly. • Track and reconcile approved business travel expenses in accordance with company policy.• All other duties as required.

Qualifications

REQUIRED SKILLS AND SPECIFICATIONS:

Technical skills: Proficiency with electronic documentation, basic office applications, and timely, accurate activity reporting; ability to learn company consent and enrollment systems. Interpersonal skills: Exceptional verbal communication, active listening, empathy, professionalism, and the ability to explain information carefully to older adults. Demonstrated patience and consistent enthusiasm during repetitive one-on-one conversations. Role-specific capabilities: Independent time management, strong attention to detail, adaptability, comfort with measurable activity goals, and willingness to travel extensively.

EDUCATION AND EXPERIENCE:

Required: High school diploma or equivalent; experience in a customer-facing, patient-facing, or community outreach role; ability to communicate clearly and document activity accurately. Preferred: Experience in skilled nursing, post-acute care, healthcare enrollment, admissions, or senior services; familiarity with patient consent processes and geriatric populations.

CERTIFICATIONS AND LICENSES:

Required: Valid driver’s license and ability to meet company travel requirements when driving is needed. No clinical license is required for this non-clinical role.

WORK ENVIRONMENT:

Field-based position with extensive travel throughout HealthDrive markets, including frequent domestic flights, overnight hotel stays, and extended periods away from home. Work is primarily performed in skilled nursing and post-acute care facilities, with administrative reporting completed electronically. Must be able to navigate facility environments, use a laptop or mobile device, and manage travel logistics independently. Annual base salary: $100,000. Approved business-related travel expenses are reimbursed in accordance with company policy.
