New Victoria Hospital, Kingston upon Thames Permanent | Full Time | 37.5 Hours per Week

Salary: £30,927 per annum

Working Hours

37.5 hours per week, working flexibly between:

08:00 and 20:00, Monday to Sunday

We're looking for a passionate and organised Connected Care Lead to join our team at New Victoria Hospital and play a pivotal role in delivering an exceptional patient experience across our Connected Care pathways.

This is an exciting new position created to support the growth of Connected Care services across the wider Bupa network. You'll work closely with consultants, clinical teams, operational colleagues, and Bupa Insurance to ensure patients receive seamless, coordinated care from referral through to treatment.

What you'll do

As Connected Care Lead, you'll become a central point of coordination for patients accessing specialist care pathways, ensuring their journey is efficient, supported and of the highest quality.

You'll:

  • Coordinate referrals, bookings and patient pathways across multiple Bupa services.
  • Act as a key liaison between patients, consultants, clinical teams, diagnostics services and Bupa Insurance.
  • Monitor patient progression and proactively resolve any pathway delays.
  • Coordinate and administer up to three specialist Multi-Disciplinary Teams (MDTs).
  • Organise MDT meetings, prepare documentation and ensure outcomes are accurately recorded and communicated.
  • Support pathway performance monitoring and service improvement initiatives.
  • Build strong relationships with internal and external stakeholders.
  • Contribute to the continued development and growth of Connected Care services.

What you'll bring

We're looking for someone who thrives in a fast-paced healthcare environment and enjoys building relationships while managing multiple priorities.

You'll ideally have:

Essential

  • Experience in healthcare administration, patient coordination, pathway management, or MDT coordination.
  • Experience working with both clinical and non-clinical teams.
  • Excellent organisational and workload management skills.
  • Strong communication and stakeholder management abilities.
  • High attention to detail and accuracy.
  • Proficiency in Microsoft Office applications and patient administration systems.
  • The ability to work independently whilst maintaining a collaborative approach.

Desirable

  • Understanding of private hospital operations.
  • Knowledge of CQC and healthcare regulatory standards.
  • Experience of healthcare performance metrics, SLAs and service improvement methodologies.
  • Degree level qualification.
