Program Overview – Community Support & Outreach - Supportive Housing, Community Care Unit
The Community Support Worker is part of the Community Supports & Outreach Team (CSO) within the Supportive Housing Department at WoodGreen Community Services. This role supports adults 18+ with mental health and/or substance use issues, concurrent disorders, and dual diagnosis. The program offers case management, recreational programming, and hoarding and pest supports within antioppressive, traumainformed, harm reduction, and mental health recovery frameworks. Clients live across four WoodGreen-owned buildings and 36 privately owned units throughout Toronto, and the role includes community-based work and travel during work hours.

The Community Support Worker reports to the Manager of Supportive Housing Programs. This position follows WoodGreen policies, procedures, and all relevant legislation, while supporting our commitment to community development, antidiscrimination, and promoting wellness and selfsufficiency. Due to WoodGreens multisite operations, staff may be relocated to another site performing the same duties based on program needs, with reasonable notice provided.

The Community Support Worker uses a clientcentered, traumainformed approach to assess tenant issues, respond to needs, deescalate crises, and provide individualized case management, counselling, and service planning. The role collaborates closely with supervisors, managers, housing staff, and external service providers to support tenant wellness, prevent evictions, mediate disputes, and strengthen community connections. The approach is inclusive, respectful, collaborative, recoveryfocused, and grounded in harm reduction.

What You Will Do

  • Deliver the Healthy Communities – Healthy Futures program, including wellness workshops, nutritional demonstrations, and health promotion clinics.
  • Complete initial and ongoing needs assessments using a strengths‑based approach.
  • Provide case management services such as goal setting, care planning, assessment, counselling, crisis intervention, follow‑up supports, and referrals.
  • Support tenants with housing stability, including move‑in/move‑out transition planning.
  • Work with Housing staff on unit‑related issues such as pest control, fire safety concerns, clutter/hoarding, and unit damages.
  • Empower clients to increase independence and achieve personal goals.
  • Support clients in building daily living skills (cooking, housekeeping, budgeting, shopping, leisure planning, etc.).
  • Identify changes in a client’s mental status or functioning and take appropriate action.
  • Provide crisis prevention, management, intervention, and conflict mediation.
  • Build and maintain supportive, flexible, ongoing relationships with clients.
  • Introduce new tenants to the community and encourage involvement in on‑site programs and engagement activities.
  • Help clients develop supportive networks and connections in the community.
  • Maintain accurate and timely documentation, case notes, and client files.
  • Document significant events, changes in service needs, and risk‑related concerns.
  • Apply ethical standards and uphold trauma‑informed, harm‑reduction, and anti‑oppressive practices.
  • Participate in team, unit, and cross‑department meetings to support healthy team functioning.
  • Work collaboratively with other WoodGreen staff to ensure effective program operations.
  • Take part in training, education, and skill‑building activities as required.
  • Support and mentor student placements and volunteers.
  • Represent the organization at external committees and forums.
  • Assist clients in navigating the health system, including hospitals, primary care, and community health centres.
  • Accompany clients to appointments that may require therapeutic support (medical appointments, court hearings, etc.).

General Program Duties

  • Provide culturally and linguistically responsive service to clients and caregivers.
  • Uphold harm‑reduction, trauma‑informed, anti‑racism, and anti‑oppressive principles in daily practice.
  • Participate in outreach to ensure ongoing access to the program.
  • Stay informed about services offered by other agencies and current best practices.
  • Provide mentorship or supervision to students and volunteers as needed.
  • Maintain an assigned caseload and deliver services through visits, email, phone, and other communication methods.
  • Complete data entry and statistical reporting in line with funder and agency requirements.
  • Represent the program on internal committees when required.
  • Be prepared to work in environments with pests, hoarding, drug use, and situations involving death and dying.
  • Perform other duties as assigned.

What You Bring to the Team

  • Bachelor’s degree in social work (BSW) or related degree.
  • Registered Social Worker (RSW) in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) and/or Member in good standing with a Professional College in Ontario strongly preferred
  • Minimum three (3) years of community experience in mental health, addictions, and/or social work
  • Related experience in case management, supportive counselling, conflict and crisis mediation and client engagement.
  • Demonstrated experience working with individuals with mental health, substance use, addiction issues, physical health issues and/or homelessness.

  • Might work in environments where clients use substances.
  • Might work in environments wherein clients have incontinence issues.
  • Clients may exhibit responsive behaviours including profanities and expletives.
  • Able to lift 40lbs.
