Community Support Professional Position Description

Beacon is a successful and national behavioral health company providing residential-based services and supports to children and adults with Complex Behavioral Health conditions, Severe Mental Illness, Autism, and other compounding medical conditions. All activities related to this position must be delivered in compliance with the company’s policies and procedures, accreditation standards, state and federal licensing requirements, HCBS guidelines, and the contractual requirements of Community Mental Health Agencies and other regulatory bodies. The Community Support Professional (CSP) provides individualized, community-based support to individuals with intellectual and developmental disabilities, empowering them to achieve greater independence and improve their quality of life. Working in the individual's home and throughout the community, the CSP assists with daily living activities, community integration, transportation, skill development, and achievement of person-centered goals outlined in each individual's service plan. This role promotes dignity, choice, and inclusion while maintaining a safe, supportive environment and ensuring services are delivered in compliance with agency standards, regulatory requirements, and documentation expectations.

Primary Responsibilities:
• Complete well written and detailed daily service notes via electronic record system. Must be able to navigate the agency’s electronic records and Electronic Visit Verification (EVV) systems. Training on EVV and electronic records systems is provided. Requires the use of personal cell phone.
• Must be willing to work with individuals in their homes, in the community and provide transportation in accordance with service delivery.
• Routine duties may include but are not limited to: assisting individuals with scheduling, attending and transportation to medical appointments, household maintenance/housekeeping, meal planning and preparation, money management and budgeting tasks, promoting problem-solving and supporting social interactions and activities. Assistance with personal hygiene and support for medication management may also be required.
• Services are delivered in accordance with the individual’s service plan and in relation to their identified outcomes. Staff are responsible for providing services as detailed in the Individual’s Service Plan.
• Promotes a safe and caring environment where individuals can learn and grow while enjoying a healthy and productive lifestyle.
• Promotes community awareness by demonstrating responsible, courteous behavior and a positive perception of individuals with developmental disabilities.
• Participates in the Interdisciplinary Team Meeting process as appropriate.
• Interacts and speaks professionally with individuals, family members, Supports Coordinators, Guardians, Supervisors and other stakeholders.

• Must be compliant with company and regulatory policies and procedures.
• Participate and contribute to weekly, bi-week or monthly leadership calls. Be prepared to report on applicable KPIs that best reflect the performance of your team.
• Preserve and protect the privacy, confidentiality and security of all medical records, proprietary and other confidential information relating to Beacon Specialized Living, its Individuals, Team Members, activities and affiliates, in accordance with all applicable laws (especially HIPAA) and contract terms.

• Follow Beacon’s policies, procedures, and manuals.
• Cooperates with Beacon Specialized Living, the Department of Human Services (DHS) staff or any other regulatory body during any inspection, investigation or inquiry.
• Accept other duties as assigned.

Education and Qualifications:
◦ High School Diploma
◦ Must pass criminal background checks, central registry check (in applicable states) and child abuse registry (in applicable states).
◦ Must complete and pass a drug test, in applicable states.
◦ Must possess a valid Driver’s License and have reliable transportation.
◦ Proficient in speaking, reading and writing the English language.
◦ Approved by state, federal and government entities to work within BSLS programs.
◦ Ability to use office equipment, including BSLS information technology systems.

While performing the duties of this job, the employee is regularly required to talk or hear. Specific vision abilities required by this job include close vision and the ability to adjust focus. This would require the ability to lift files, open filing cabinets, and bend or stand on a stool as necessary.
Position Type/Expected Hours of Work:
This position requires flexibility in scheduling and travel to various locations within Lackawanna and Luzerne counties as well as surrounding communities.
Scheduling needs may include varying hours and days, including nights, weekends and holidays in order to meet the needs of the individuals we serve.
