The Community Supports Professional (CSP) is responsible for supporting the needs of the individual in the In-Home and Community Support, and Companion Services programs. The position requires the CSP to assist individuals with the performance and execution of daily living requirements necessary to maintain their residence. The CSP will work with the individual's support team to identify and assess personal outcomes as defined by the individual's goals and support plan The CSP will also conduct training on how to perform life skills such as cleaning, cooking, budgeting, and personal care. The CSP provides transportation to the individual as needed. The CSP documents all visits and activities in the Electronic Health Records System (EHR). The CSP utilizes any required vehicle verification system to document and track specific visit locations and durations as required. The CSP may work with other members of the individual's support team, including other staff, friends, family members, and/or community support members to design training, support, and interventions to ensure the success of the individual and execution of the individual's plan and goals. The CSP must maintain clear and accurate documentation as required by the Program Specialist to ensure quality service delivery. The CSP reports to the Program Specialist - In-Home and Community Supports.
