Salary Range: $18.50 - $18.50 Hourly, The Community Support Liaison (CSL) serves as a liaison between the Crisis Response Center (CRC) and Outpatient Treatment Teams to facilitate communication, coordination, continuity of care, and discharge planning. The CSL completes urgent enrollments for COPE while patients are hospitalized, ensuring timely connection to appropriate services and supports. The CSL provides flexible coverage across COPE Residential sites as part of the CSL Team, offering assistance and support based on program and patient needs.Qualifications: MINIMUM EDUCATION/EXPERIENCE QUALIFICATIONS:
Must be 21 years of age (automobile insurance requirement) and possess a valid AZ driver’s license with a “clean” or “acceptable” driving record. An unacceptable driving record includes (but is not limited to) the following:
Other driving related violations may be considered unacceptable. At time of job offer, submission of 36 month (three year) driving record from DMV will be required. These standards must be maintained throughout employment and up-to-date driving records are reviewed regularly for current employees.
