Join our team at Florida Parishes Human Services Authority as a Community Service Professional 3. This position is domiciled in Hammond, Louisiana, and is in our Developmental Disabilities Services division. This position plays a vital role in determining eligibility, coordinating services, and connecting individuals to critical community-based supports and resources. We are seeking a compassionate, detail-oriented professional with strong knowledge of developmental disabilities services, waiver programs, and community resources.

Examples of duties

Six years of experience in providing health or developmentally disabled services, counseling, health care management, or social services; OR

Six years of full-time work experience plus three years of experience in providing health or developmentally disabled services, counseling, health care management, or social services; OR

A bachelor's degree plus three years of experience in providing health or developmentally disabled services, counseling, health care management, or social services; OR

An advanced degree in a social sciences field, nursing, public health; public, hospital, or health administration; speech-language pathology or communication sciences and disorders; physical, occupational, recreational, or music therapy plus two years of experience in providing health or developmentally disabled services, counseling, health care management, or social services.

EXPERIENCE SUBSTITUTION:
Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field.

Supplemental information

Job Duties:

  • Provides information and referrals to individuals and families regarding available programs, services, and community resources.
  • Coordinates with families, schools, and professionals to obtain documentation needed for eligibility and service determinations.
  • Conducts face-to-face interviews and administers assessments to evaluate support needs and makes referrals to Family Support programs, Resource Centers, community agencies, and other appropriate providers.
  • Maintains the Participant Services Database, including demographic information, eligibility outcomes, and requested services.
  • Ensures individuals and families are informed of services available through DDS, Medicaid, and local/state agencies.
  • Determines an individual’s level of need using required assessment tools and participates in assigning priority levels for services.
  • Independently submits referrals for waivers, prepares eligibility appeal documentation, represents the agency during appeal hearings by presenting supporting evidence; and
  • Reviews and evaluates Screening for Urgency of Need (SUN) requests, including initial screenings and re-screenings, and submits requests to OCDD as appropriate.

Compensation: The salary range for the position is below.

  • Community Service Professional 3 : $22.96/hour minimum - $41.38/hour maximum

Salary may be negotiable depending on the chosen applicant’s level of experience. Note regarding the advertised pay range: The maximum amount listed is the maximum salary a person can make while in this title and NOTthe maximum amount we are allowed to pay a new hire.
