Job Description

Job
Title:
Home Health Sales Liaison

Company: Home Health Care, Inc.
Location: Greater Twin Cities Metro Area
Compensation:
$100,000 to $130,000 base + uncapped commission

Schedule: Full-Time, Monday through Friday

To
stand out, email
tom.lecy@hhcare.net

with your resume and a brief summary of your

weekly activity metrics,
referral volume, conversion rates, and examples of cold accounts you turned
into active business

.

This
is a
high-energy, field-based sales role built for a true hunter

who
wants to compete in one of the most active and relationship-driven spaces in
healthcare. You will be out in the market daily calling on hospitals,
transitional care units, skilled nursing facilities, and clinics to identify
patients discharging home and position Home Health Care, Inc. as the preferred
provider. In a market where organizations like Interim, Adara,
Lifespark

, and large health systems often rely on brand
recognition and inbound referrals, this role is built for someone who
creates
opportunity, builds new referral channels, and wins business through
consistency, responsiveness, and execution

.

Earnings
potential and book-of-business growth:

  • Base salary gives you stability while you build and
    expand your territory.

  • Based on prior-year sales performance, commission
    earnings have ranged from approximately
    $41,000 to $90,000+ annually

    for established activity levels.

  • Average annual commission earnings have been around
    $59,000

    ,
with higher upside for consistent producers.

  • As your referral sources mature, your book of business
can continue to compound year over year.

  • Strong performers have the ability to reach total
    annual earnings in the
    $140K to $180K+ range

    through consistent
    field activity and conversion.

Why
this opportunity is different:

  • You represent a
    32-year established provider

with credibility across the Twin Cities.

  • You are not just maintaining accounts, you are expected
    to
prospect, open doors, and create new referral flow

    .

  • You will have strong support from intake, clinical
leadership, and operations to help convert referrals smoothly.

  • You will compete in the market against providers such
as Allina Home Care, Fairview Home Care, Interim HealthCare, BrightStar
Care, and other local home health providers.

  • Many competitor postings talk broadly about
“relationship building,” but this role is built around
    measurable
    activity, referral growth, and admissions

    .

What
you will be responsible for:

  • Prospecting new referral sources through daily field
visits, cold outreach, drop-ins, and local networking.

  • Building trusted relationships with discharge planners,
case managers, physicians, social workers, and facility leadership.

  • Educating referral partners on home health eligibility,
service lines, clinical responsiveness, and transition support.

  • Creating warm handoffs for patients and families who
    need skilled services after discharge.

  • Partnering with intake and clinical teams to remove
    barriers and improve referral-to-admission conversion.

  • Logging activity, tracking pipeline movement, and using
    data to improve territory performance.

  • Staying ahead of competitors by being more responsive,
    more visible, and more consistent.

What
we are looking for:

  • Bachelor’s degree in Business Administration or related
    field is required.

  • Medical device, pharmaceutical, DME, home health,
hospice, or directly related healthcare sales experience strongly
    preferred.

  • Proven hunter mentality with examples of cold leads
    converted into active referral accounts.

  • Ability to speak clearly about production, including
touchpoints, referral volume, conversion rates, and revenue or admission
    outcomes.

  • Strong comfort level calling on hospitals, SNFs, TCUs,
    clinics, and long-term care communities.

  • Professional presence, strong follow-up, and confidence
    influencing clinical and non-clinical decision-makers.

  • Valid driver’s license and willingness to travel daily
    throughout the territory.
