About the Role:

Juniper is seeking experienced Registered Nurses to join our Home and Community Care team supporting clients across the Perth metro area.

In this role, you will provide clinical care and nursing support to clients living independently in the community. You will conduct assessments, develop and review care plans, and work collaboratively with clients, families and healthcare professionals to ensure safe, high-quality care at home.

Juniper is currently seeking 2x Community Registered Nurses to join the team. Please see position details:

• 1x casual contract; the successful candidate must be available Wednesdays and 1 weekend day a week.
• 1x permanent part-time contract; the successful candidate must be available Monday-Wednesday 8am-4.30pm.

Key Responsibilities:

• Conduct clinical assessments and respond to changes in client health and wellbeing
• Develop, implement and review person-centred care plans with clients and their families
• Provide clinical care including medication management, wound care and health monitoring
• Support clients to maintain independence and wellbeing in their own homes
• Liaise with GPs, allied health professionals and other service providers to coordinate care
• Provide clinical advice and support to community care staff
• Maintain accurate and timely clinical documentation and reporting
• Ensure compliance with organisational policies and Aged Care Quality Standards
• Participate in quality improvement and safe clinical practice initiatives

Key Requirements:

• Current AHPRA registration as a Registered Nurse
• Experience in aged care, community care or a similar clinical setting
• Strong clinical assessment and care coordination skills
• Ability to work independently in a community-based role
• Excellent communication, organisation and problem-solving skills
• Current driver’s license and willingness to travel between client homes
• Current National Police Clearance (or willingness to obtain)
