Job Description


This is a hybrid role that blends clinical social work expertise with relationship‑based community outreach; This position supports clients and families navigating care decisions while building trusted relationships with referral partners to drive appropriate private‑duty care placements; The role serves as a bridge between client needs, family decision‑making, and community partnerships, ensuring clients receive personalized, high‑quality care while supporting appropriate census growth through education, advocacy, and care coordination as outlined in the marketing plan for EverTrue Anywhere Care.


Essential Duties and Responsibilities:

1. Conducts psychosocial assessments to identify client needs, risks, and support systems and creates care plans to help clients meet their goals

2. Assists families in navigating care decisions, transitions, and long‑term planning

3. Identifies barriers to care (financial, emotional, social, logistical) and helps develop solutions

4. Educates families on private duty care options promoting EverTrue as the Anywhere Care services provider of choice in the metro St. Louis area with special emphasis on home help, At Home +, and GUIDE

5. Manages client enrollment and executes contracts for new clients

6. Builds and maintains relationships with referral sources including hospitals and discharge planners, skilled nursing facilities, physicians and clinics, senior living communities, elder law attorneys, fiduciaries, and community organizations

7. Serves as a trusted clinical resource educating partners about the scope of private duty services through EverTrue Home Help, At Home + and GUIDE services

8. Facilitates introductions and warm hand‑offs between referral partners and intake teams supporting timely service initiation

9. Establishes a positive image of EverTrue in the public community representing the organization at community meetings, networking events, and educational sessions

10. Assists families in understanding care recommendations, pricing, and next steps, addressing hesitations or concerns by providing empathetic, clinical guidance

11. Supports conversion from inquiry to active service by establishing trust and clarity, following up on pending inquiries to ensure continuity and responsiveness

12. Supports census growth through ethical, relationship‑based outreach balancing advocacy for client needs with responsible business sustainability

13. Tracks referral activity, outcomes, and pipeline progress in the EverTrue database throughout the marketing and sales process daily sharing insights and partnership feedback with leadership

14. Participates in weekly sales and marketing meetings with the Anywhere Care team

15. Stays current on, and applies a firm understanding of, Medicare and Medicaid regulations related to the various programs and services; current insurance contracts across the EverTrue continuum of care

16. Maintains a high standard of excellence by promoting a positive, productive, and compassionate customer experience, always striving to exceed customer expectations, and treating patients, residents, guests, clients, families, physicians, visitors, vendors, and coworkers with dignity and respect

17. Connects clients and customers to the entire EverTrue continuum of care as appropriate

18. Promotes an atmosphere of teamwork, emphasizing a group effort in which the contribution of every employee is essential

19. Participates in weekly sales and marketing meetings with the Anywhere Care team

20. Meets with the Anywhere Care Director of Sales and Marketing and Home Help and Answers leadership to review and develop quarterly goals and action plans

Knowledge, Skills, and Abilities:

  • Bachelor’s degree in Social Work (BSW) with sales experience; Master of Social Work (MSW) preferred

  • Certified Care Manager preferred

  • Requires 5+ years of prior healthcare community liaison experience, with demonstrated success in referral and business development in home care or senior services.

  • Excellent speaking, writing, and sales skills

  • Strong interpersonal, presentation, and relationship‑building skills

  • Self-motivation to work independently and as a team member

  • Advanced knowledge of computer technology; Database experience

  • Must possess and demonstrate skills and ability to effectively present information to management and public groups

  • Ability to interact effectively with referral sources, EverTrue team members, residents, clients, families, visitors, and the public

  • Ability to implement and interpret programs, goals, objectives, policies, and procedures.

  • This position will operate independently and will require frequent local travel to generate appropriate referrals and program enrollments with emphasis on private duty, At Home +, and GUIDE.

  • The individual must be comfortable in a clinical environment and have a general knowledge of medical terminology

A well-lighted and ventilated working area that has temperature control system (air conditioning and heat); The work will have some short notice requests, timelines for completion of tasks or reports and some major projects extending over months; The employee will experience frequent travel by car with occasional overnight stays. Some exposure to outside weather conditions in the course of travel; Minimal exposure to infectious diseases and blood borne pathogens; Minimal exposure to chemicals and hazardous waste.

EverTrue Pay & Benefits

Pay grade: E ($62,300.00/year to $99,900.00/year)
